腹腔镜联合胆道镜行保胆取石治疗胆囊结石的可行性及安全性

摘要目的探讨腹腔镜联合胆道镜行保胆取石治疗胆囊结石的可行性及安全性。方法选取2014年8月～2016年3月我院收治的78例胆囊结石患者作为研究对象，按照患者就诊ID号奇、偶数将其分为观察组与对照组，各39例。对照组行胆道镜治疗，观察组行腹腔镜联合胆道镜治疗。比较两组的手术治疗指标以及术后恢复指标。结果观察组的手术时间显著长于对照组，差异有统计学意义（P＜0.01）。观察组的术中出血量显著少于对照组，饮食恢复时间、胃肠功能恢复时间、住院时间显著短于对照组，差异有统计学意义（P＜0.01）。两组的并发症发生率比较，差异无统计学意义（P＞0.05）。观察组术后1、6、12个月的胆囊壁厚度以及胆囊收缩功能均优于对照组，差异有统计学意义（P＜0.01）。观察组术后1、6、12个月的生活质量评分显著高于对照组，差异有统计学意义（P＜0.01）。结论应用腹腔镜联合胆道镜行保胆取石治疗胆囊结石安全可行，可促进胆囊功能恢复，提高患者术后生活质量，值得临床推广应用。

Abstract：Objective To explore the feasibility and safety of laparoscope combined with choledochoscope for preservation of gallbladder in the treatment of cholecystolithiasis.M ethods 78 patients with cholecystolithiasis from August 2014 to March 2016 in our hospitalwere selected and divided into the control group and the observation group according to the ID number of odd and even numbers,39 cases in each group.The control group was treated with choledochoscope,the observation group was treated with laparoscope combined with choledochoscope.The surgical indexes and postoperative recovery indicators were compared between the two groups.Results The operation time of the observation group was longer than that of the control group,with significant difference(P＜0.01).The amount of bleeding in the observation group was less than that in the control group,the diet recovery time,the gastrointestinal function recovery time, hospitalization time in the observation group was shorter than that in the control group,with significant difference(P＜0.01).There was no significant difference in the incidence rate of complication between the two groups(P＞0.05).The gallbladderwall thickness and gallbladder contractile function at 1,6,12months after operation in the observation group was better than that in the control group,with significant difference(P＜0.01).The scores of quality of life at 1,6,12 months after operation in the observation group was higher than that in the control group,with significant difference(P＜0.01).Conclusion The application of laparoscope combined with choledochoscope for preservation of gallbladder in the treatment of cholecystolithiasis is safe and feasible,which can promote the recovery of gallbladder function and improve the quality of life of patients after surgery,it isworthy of clinical popularization and application.
