经鼻持续气道正压通气联合丙种球蛋白治疗儿童重症毛细支气管炎的效果观察

摘要目的探讨经鼻持续气道正压通气联合丙种球蛋白治疗儿童重症毛细支气管炎的效果。　方法选取本院2013年10月～2015年10月收治的70例重症毛细支气管炎患儿作为研究对象，根据治疗方法的不同分为观察组及对照组。观察组采用经鼻持续气道正压通气联合丙种球蛋白治疗，对照组采用常规吸氧方式及对症治疗，比较两组的治疗效果。　结果观察组的住院时间、喘憋缓解时间及总吸氧时间显著短于对照组，差异有统计学意义（P＜0.05）。观察组治疗2 h后的呼吸频率、心率显著低于对照组，TcSO2水平显著高于对照组，差异有统计学意义（P＜0.05）。观察组治疗24 h后的血气指标改善程度显著优于对照组，差异有统计学意义（P＜0.05）。观察组的并发症发生率显著低于对照组，差异有统计学意义（P＜0.05）。　结论经鼻持续气道正压通气联合丙种球蛋白治疗可改善婴儿重症毛细支气管炎的通气、换气功能，减轻免疫损伤，快速缓解症状，缩短住院时间。

Abstract：Objective To explore the effect of nasal continuous positive airway pressure（nCPAP）combined with gamma globulin in the treatment of severe capillary bronchitis in children.Methods 70 children with severe capillary bronchitis in our hospital from October 2013 to October 2015 were selected and divided into the observation group and the control group.The observation group was given nCPAP combined with gamma globulin，the control group was given routine oxygen inhalation and symptomatic treatment.The effect of the two groups was compared.Results The hospital stay，the remission time of asthma and the overall time of oxygen inhaling in the observation group was shorter than that in the control group，with significant difference　（P＜0.05）.The respiratory rate and heart rate of the observation group after treatment of 2 hours was lower than that of the control group，the level of TcSO2in the observation group after treatment of 2 hours was higher than that in the control group，with significant difference　（P＜0.05）.The improvement degree of blood gas indexe in the observation group after treatment of 24 hours was better than that in the control group，with significant difference（P＜0.05）.The incidence rate of complication in the observation group was lower than that in the control group，with significant difference　（P＜0.05）.Conclusion nCPAP combined with gamma globulin can improve the ventilation and gas exchange function in children with severe capillary bronchitis，reduce immune injury，relieve the symptoms rapidly，and shorten hospital stay.
