Correlation between micro-inflammation status and nutritional status and cardiac function in maintenance hemodialysis patients with end-stage renal disease

Abstract:Objective To explore the relationship between the micro-inflammatory state of maintenance hemodialysis in patients with end-stage renal disease (ESRD) and heart failure, nutritional status, and cardiac function.Methods Clinical data of 60 patients with ESRD admitted to the First People′s Hospital of Fuzhou City, Jiangxi Province from February 2018 to April 2020 were retrospectively analyzed.According to different treatment methods, the patients were divided into two groups: 30 patients receiving maintenance HD were treated as the HD group, and 30 patients without dialysis were treated as the non-dialysis group.30 healthy people who underwent physical examination in First People′s Hospital of Fuzhou City, Jiangxi Province were selected as the control group.According to the New York Heart Association cardiac function (NYHA) classification standard classification method, 60 patients with ESRD were rated Ⅱ20 cases,Ⅲ23 cases,Ⅳ17 cases.Fastening venous blood of each group was collected,and its C-reactive protein(CRP),hemoglobin (Hb), albumin (ALB), urea nitrogen (BUN), serum creatinine (SCr), B-type natriuretic peptide (BNP), serum iron (SF), etc., and the left ventricular ejection fraction (LVEF) of the three groups was measured by cardiac color Doppler ultrasonography.Results The CRP levels in the non-dialysis group and HD group were higher than those in the control group, and the differences were statistically significant (P<0.05).The CRP level in HD group was higher than that in non-dialysis group, and the difference was statistically significant (P<0.05).There was no correlation between CRP level and SF and SCr level in non-dialysis group and HD group (r=0.16, 0.24, P>0.05).The CRP level in ESRD patients was negatively correlated with ALB and HB (r=-0.50,-0.36, P<0.05).The levels of CRP and BNP in the grade Ⅱgroup were lower than those in the grade Ⅲand Ⅳgroups, and the differences were statistically significant (P<0.05).The levels of CRP and BNP in the grade Ⅲgroup were lower than those in the grade Ⅳgroup,and the differences were statistically significant (P<0.05).The LVEF of the grade Ⅱgroup was higher than that of the grade Ⅲand Ⅳgroup, and the differences were statistically significant (P<0.05).The LVEF of the grade Ⅲgroup was higher than that of the grade Ⅳgroup, and the differences were statistically significant (P<0.05).The CRP level in ESRD patients was negatively correlated with LVEF (r=-0.36, P<0.05), and positively correlated with plasma BNP (r=0.58, P<0.05).Conclusion For ESRD patients and heart failure, most of them present in a micro-inflammatory state,and this state is closely related to malnutrition and cardiac function.
