Current status and influencing factors of the return to work in young and middle-aged patients with spinal cord injury

Abstract:Objective To explore the status and influencing factors of the return to work in young and middle-aged patients with spinal cord injury. Methods A total of 165 young and middle-aged patients with spinal cord injury discharged from Luoyang Orthopaedic-Traumatological Hospital of Henan Province (Henan Orthopaedic Hospital) from January 2018 to 2020 as the research objects. Counted the patients′ return to work and analyzed their relevant influencing factors. Results Fifty-three (32%) patients returned to work and one hundred-twelve (68%) are currently unemployed. The results of the univariate analysis showed that,cultural level (χ2=6.303,P=0.043),damage site (χ2=10.788,P=0.005),American Spinal Injury Association (ASIA) grade (χ2=52.177,P<0.001),mentality (χ2=53.646,P<0.001),return to work (χ2=130.143,P<0.001),Barthel score (t=6.764,P<0.001) was a associated factor affecting the return to work in young and middle-aged spinal cord injury patients. Gender,age,marital status,method of payment,presence of work before injury,chronic history,postoperative time of both groups,and hospital stay days were compared between the two groups,the differences were not significantly significant (P>0.05). The results of the multivariate analysis showed that the Barthel score (OR=0.940,95%CI=1.042-1.102,P<0.001),ASIA grade (OR=5.782,95%CI=1.109-39.159,P=0.037),patient mentality (OR=6.336,95%CI=1.522-26.375,P=0.011),the pathway of return to work (OR=56.503,95%CI=9.468-337.184,P<0.001) were independent risk factors affecting young and middle-aged spinal cord injury patients. Conclusion During the long recovery process of young and middle-aged spinal cord injury patients,medical staff can help patients to return to work by improving their cognition and confidence in returning to work,and providing some ways to return to work.
