Diagnostic value of T cell spot test of tuberculosis infection of pleural effusion and peripheral blood in tuberculous pleural effusion |

Abstract Objective To explore the diagnostic value of T cell spot test of tuberculosis infection (T-SPOT.TB) of pleural effusion and peripheral blood in tuberculous pleural effusion.Methods A total of 80 patients with unexplained pleural effusion who were admitted to Shenzhen Hengsheng Hospital from January 2017 to December 2018 were selected as the research objects.Peripheral blood and pleural fluid samples were collected from patients,and mononuclear cells were separated with sterile Ficoll lymphocyte fluid,T-SPOT.TB was performed,and the test results were analyzed.Those with positive test results of gold standard were tuberculous pleural effusion group,and those with negative test results were non-tuberculous pleural effusion group.The diagnostic value of T-SPOT.TB test in peripheral blood and pleural effusion were analyzed.Results There were 47 cases with positive results and 33 cases with negative results according to gold standard.The positive rates of T-SPOT.TB and Adenosine Deaminase (ADA) in pleural effusion and peripheral blood in tuberculous pleural effusion group were higher than those in non-tuberculous pleural effusion group,and the differences were statistically significant (P<0.05).The positive rate of T-SPOT.TB detection in patients with tuberculous pleural effusion group was higher than that in peripheral blood,and the difference was statistically significant (P<0.05).The positive rates of T-SPOT.TB detection in pleural effusion and peripheral blood were higher than that of ADA detection,and the differences were statistically significant (P<0.05).The sensitivity,specificity,positive predictive value and negative predictive value of T-SPOT.TB test in the diagnosis of tuberculous pleural effusion were higher than those of T-SPOT.TB test in peripheral blood,and the differences were statistically significant (P<0.05).Conclusion The detection of T-SPOT.TB in peripheral blood and pleural effusion can be used as an auxiliary means for the diagnosis of tuberculous pleural effusion,but compared with peripheral blood,the detection of mononuclear cells in pleural effusion is more sensitive and can be widely used in clinic.
