Application of ultrasound-guided thoracic paravertebral block technique in thoracic surgery |

Abstract Objective To investigate the application effect of ultrasound-guided thoracic paravertebral block technique in thoracic surgery.Methods A total of 60 patients who underwent thoracotomy in Xinyu People′s Hospital from January 2019 to December 2020 were selected and divided into general anesthesia group(group A)and general anesthesia combined ultrasound-guided thoracic paravertebral block group (group B)by random number table method,with 30 cases in each group.The hemodynamics,the number of post-anesthesia care unit(PACU)compressions,the degree of pain and adverse reactions in different postoperative states were compared between the two groups.Results After entering the room and after anesthesia intubation,there was no statistically significant difference of the mean arterial pressure(MAP)between the two groups(P>0.05).At thirty minutes after the operation and at the end of the operation,the MAP of the group B was lower than that of the group A in the same period,with statistically significant difference (P<0.05).At thirty minutes after the operation and at the end of the operation,the MAP of the group A was higher than that after entering the room and after anesthesia intubation in the group,with statistically significant difference (P<0.05).There was no statistically significant difference of the MAP in the group B at various time points(P>0.05).After entering the room,there was no statistically significant difference of the heart rate (HR)between the two groups (P>0.05).After anesthesia intubation,at thirty minutes after the operation and at the end of the operation,the HR of the group B was higher than that of the group A in the same period,with statistically significant difference (P<0.05).After anesthesia intubation,at thirty minutes after the operation and at the end of the operation,the HR of the group A was higher than that after entering the room and after anesthesia intubation in the group,with statistically significant difference (P<0.05).There was no statistically significant difference of the HR in the group B at various time points (P>0.05).The PACU presses in the group B was less than that in the group A,the resting state and activity state scores in the group B were lower than those in the group A,the differences were statistically significant(P<0.05).The total incidence of adverse reactions in the B group was lower than that in the A group,and the difference was statistically significant(P<0.05).Conclusion General anesthesia combined with ultrasound-guided thoracic paravertebral block anesthesia for patients undergoing thoracic surgery has a good analgesic effect,which is beneficial to stabilize hemodynamics,reduce the number of PACU compressions,and reduce the incidence of adverse reactions.It is worthy of extensive application.
