Abstract Objective To explore the application effect of transitional care(TCM)model in the transfer of stroke patients from intensive care unit(ICU)to general ward. Methods A total of 200 stroke patients who were transferred from the ICU to the general ward in the First Affiliated Hospital of Nanchang University from January 2018 to September 2020 were selected and numbered according to the order of entry. The even group was set as the test group(100 cases)and the odd group was set as the control group(100 cases). The control group carried out routine care, and the test group adopted TCM mode on the basis of routine care. The self-care ability, family members′ management effect of bed rest and nursing satisfaction were compared between two groups. Results There was no statistically significant difference in the self-care ability scale(ESCA)scores of the two groups before intervention(P>0.05);the ESCA score of the test group after intervention(72.87±6.33)was higher than that of the control group of(55.48±5.15)points,the good rate of bed rest management effect(92.00%)and nursing satisfaction(97.00%)were higher than those of the control group(71.00%,82.00%),and the differences were statistically significant(P<0.05).Conclusion The TCM model can effectively improve the self-care ability of stroke patients transferred from the ICU to the general ward and the ability of their families to manage the patients in bed,and achieve high satisfaction.
