高流量湿化氧疗在慢性阻塞性肺疾病机械通气患者撤机后的序贯应用

Abstract Objective To investigate the sequential application effect of high-flow nasal cannula(HFNC)in patients with chronic obstructive pulmonary disease(COPD)after mechanical ventilation weaning.Methods A total of 82 COPD patients who were treated by mechanical ventilation from the Department of severe medicine,People′s Hospital of Jiangmen from January 2019 to October 2020 were selected as the research objects,and they were divided into the experimental group and the control group according to the order of admission time,with 41 cases in each group.The experimental group was connected with HFNC after withdrawal,and the control group was given oxygen inhalation with medium flow in the endotracheal tube after withdrawal.The respiratory rate(RR),heart rate(HR),arterial oxygen saturation(SaO2),arterial partial pressure of oxygen(PaO2),arterial partial pressure of carbon dioxide(PaCO2),incidence of secondary mechanical ventilation and time of extubation were compared between the two groups after treatment.Results After 12,24 h of treatment,there was no significant difference in RR and HR between the two groups(P＞0.05).The indexes of SaO2 and PaO2 in both groups after 24 h of treatment were higher than those after 12 h of treatment,and the differences were statistically significant(P＜0.05);PaCO2 index in the experimental group after 24 h of treatment was lower than that after 12 h of treatment,and the difference was statistically significant(P＜0.05).The total incidence of secondary mechanical ventilation in the experimental group was lower than that in the control group,and the time of extubation in the experimental group was shorter than that in the control group,the differences were statistically significant(P＜0.05).Conclusion In the clinical diagnosis and treatment of COPD patients with mechanical ventilation after weaning sequential treatment,HFNC is more helpful to reduce the incidence of secondary mechanical ventilation and achieve the goal of early extubation,which is worthy of clinical application.
