Abstract ObjectiveTo explore the effect of case management model in the management of patients with chronic heart failure.MethodsAltogether 100 cases of patients with chronic heart failure who were treated in Department of Cardiology of our hospital from January 2014 to August 2015 were selected and randomly divided into two groups;control group received the conventional management mode during the course of treatment;while,experiment group received case management mode during the course of treatment.The difference between the management of patients with chronic heart failure of two groups were compared.ResultsThe total satisfaction degree of experiment group (96.00%)was significantly higher than that of control group(82.00%),and the differences were statistically significant(P<0.05).Before intervention, there were no significantly different of SDS,SAS scores between two groups(P>0.05);after intervention,the scores of SDS,SAS of two groups were all decreased and compared with those before intervention,the differences were statistically significant(P<0.05),and those in experiment group was lower than those in control group,and the differences were statistically significant(P<0.05).According to the cardiac examination results,the recovery time and hospitalization time of experiment group were significantly shorter than those of control group,and the differences were statistically significant (P<0.05).ConclusionImplementation of case management model in the management of patients with chronic heart failurecan effectively improve the patients′psychological state and satisfaction degree,shorten the recovery time,which is of great application value.
