Influence of recommended energy in Chinese Dietary Reference Intakes(2013 Edition) on the weight, blood glucose and pregnancy outcome of pregnant women with gestational diabetes mellitus

Abstract: Objective To explore the effect of recommended energy in Chinese Dietary Reference Intakes (2013 Edition) (2013 DRIS) on the weight, blood glucose and pregnancy outcome of pregnant women with gestational diabetes mellitus (GDM). Methods A total of 482 pregnant women who were diagnosed with GDM in the Obstetric Nutrition Clinic of Dalian Maternal and Child Health Hospital from January 2018 to May 2019 were selected and were given medical nutrition therapy (MNT) both. They were divided into experimental group (n=248) and control group (n=234)according to the order of visits. In the experimental group, the recommended energy of 2013 DRIS was used, and in the control group, the recommended energy of Guidelines for Diagnosis and Treatment of Diabetes in Pregnancy 2014 was used. Follow up until delivery. Weight gain, blood glucose level, blood glucose control during pregnancy, pregnancy outcome, neonatal condition, and total incidence of delivery complications were compared between the two groups. Results The weight increase of pregnant women in the experimental group was greater than that of the control group, and the total protein and albumin levels after intervention were higher than those of the control group, with statistically significant differences (P<0.05). Fasting blood glucose and blood glucose 2 h after meal in both groups after intervention were lower than those before intervention, and the differences were statistically significant (P<0.05). There were no statistically significant differences between the two groups in fasting blood glucose, blood glucose 2 h after meal and glycosylated hemoglobin after intervention (P>0.05). The incidence of premature birth in the experimental group was lower than that in the control group, and the difference was statistically significant (P<0.05). The incidence of low birth weight infants in the experimental group was lower than that in the control group, and the difference was statistically significant (P<0.05). There were no statistically significant differences between the two groups in blood glucose control pass rate, urine ketone positive rate, newborn birth weight, total incidence of delivery complications, cesarean section rate, and incidence of macrosomia (P>0.05). Conclusion The use of energy recommended by 2013 DRIS for pregnant women with GDM is more conducive to reasonable weight gain during pregnancy, improving the nutritional status of pregnant women, reducing the incidence of preterm and low birth weight infants, without affecting the effect of blood sugar control.
