丙泊酚与依托咪酯分别复合舒芬太尼在舌系带成形术麻醉中的效果比较

摘要目的比较丙泊酚与依托咪酯分别复合舒芬太尼在舌系带成形术麻醉中的效果。方法选择2018年6～10月在昆明市儿童医院择期行舌系带成形术的110例ASA分级Ⅰ～Ⅱ级的患儿作为研究对象，按照随机数字表法将其分为丙泊酚-舒芬太尼静脉复合麻醉组（P组）和依托咪酯-舒芬太尼静脉复合麻醉组（E组），每组各55例。记录两组患儿麻醉诱导前（T1）、麻醉诱导后（T2），手术时（T3）、苏醒时（T4）的血流动力学指标[心率（HR）、收缩压（SBP）、舒张压（DBP）、平均动脉压（MAP）、脉搏血氧饱和度（SpO2）]，比较两组患儿的麻醉诱导时间、手术时间、苏醒时间以及不良反应总发生率。结果两组患儿的麻醉诱导时间、手术时间比较，差异无统计学意义（P＞0.05）；E组患儿的苏醒时间短于 P 组，差异有统计学意义（P＜0.05）。两组患儿 T1、T4的 DBP、SBP、MAP、HR、SpO2比较，差异无统计学意义（P＞0.05）；两组患儿 T2、T3、T4的 DBP、SBP、MAP、HR、SpO2均低于 T1，差异有统计学意义（P＜0.05）；E组患儿 T2、T3的 DBP、SBP、MAP、HR、SpO2均高于 P 组，差异有统计学意义（P＜0.05）。 E 组患儿的不良反应总发生率为54.5%，低于P组的90.9%，差异有统计学意义（P＜0.05）。结论依托咪酯复合舒芬太尼用于儿童舌系带成形术血流动力学稳定，麻醉平稳，术中不良反应少，术后苏醒舒适，安全性好。

Abstract：Objective To compare the effect of Propofol and Etomidte combined Sufentanil in anesthesia of lingual frenoplasty.Methods A total of 110 children with ASAⅠ-Ⅱ who underwent elective lingual frenoplasty in Kunming Children′s Hospital from June to October 2018 were selected as subjects.They were divided into Propofol-Sufuftanil intravenous anesthesia group (group P)and Etomidate-Sufuftanil intravenous anesthesia group (group E)according to the random number table method,55 cases in each group.The hemodynamic parameters(heart rate[HR],systolic blood pressure [SBP],diastolic blood pressure [DBP],mean arterial pressure [MAP],pulse oxygen saturation [SpO2])were recorded before the induction of anesthesia(T1),after anesthesia induction(T2),during surgery(T3),and waking up(T4).The anesthesia induction time,operation time,recovery time,and total incidence rate of adverse reactions were compared between the two groups.Results There were no significant differences in anesthesia induction time and operation time between the two groups(P＞0.05).The recovery time of children in group E was shorter than that in group P,and the difference was statistically significant(P＜0.05).There were no significant differences in DBP,SBP,MAP,HR and SpO2between the two groups at T1and T4(P＞0.05).The DBP,SBP,MAP,HR and SpO2at T2,T3and T4in the two groups were lower than those at T1,and the differences were statistically significant(P＜0.05).The DBP,SBP,MAP,HR and SpO2at T2and T3in group E were higher than those in group P,and the differences were statistically significant(P＜0.05).The total incidence rate of adverse reactions in group E was 54.5%,which was lower than that in group P(90.9%),and the difference was statistically significant(P＜0.05).Conclusion The Etomidte combined with Sufentanil has stable hemodynamics,with smooth anesthesia,less intraoperative adverse reactions,comfort postoperative recovery,and good safety.
