软通道辅助下微骨窗开颅血肿清除术治疗基底节区脑出血合并脑疝的观察

摘要目的探讨软通道辅助下微骨窗开颅血肿清除术治疗基底节区脑出血合并脑疝的疗效。方法选取本科2013年12月～2015年12月收治的40例基底节区脑出血合并脑疝患者作为研究对象，将2014年12月～2015年12月采用软通道辅助下微骨窗开颅血肿清除术治疗的16例基底节区脑出血合并脑疝患者作为治疗组，将2013 年12月～2014年11月采用大骨瓣开颅血肿清除加去骨瓣减压术治疗的24例基底节区脑出血合并脑疝患者作为对照组。比较两组的血肿清除率、手术时间、颅骨缺损面积以及临床有效率、死亡率。结果两组的血肿清除率、死亡率、临床有效率比较，差异无统计学意义（P＞0.05）。治疗组的手术时间显著短于对照组，术后颅骨缺损面积显著小于对照组，差异有统计学意义（P＜0.05）。结论软通道辅助下微骨窗开颅血肿清除术可作为基底节区脑出血合并脑疝患者优先考虑的手术方式。

Abstract：Objective To explore the effect of hematoma removal surgery of soft-channel-assisted micro-bone window craniotomy in the treatment of basal ganglia hemorrhage complicated with cerebral hernia.Methods 40 patients with basal ganglia hemorrhage complicated with cerebral hernia from December 2013 to December 2015 in our department were selected as the research object，16 cases of basal ganglia hemorrhage combined with cerebral hernia from December 2014 to December 2015 underwent hematoma removal surgery of soft-channel-assisted micro-bone window craniotomy were selected as the treatment group，24 cases of basal ganglia hemorrhage combined with cerebral hernia underwent large bone flap craniotomy hematoma removal combined with bone flap decompression from December 2013 to November 2014 were selected as the control group.The hematoma clearance rate，operation time，skull defect area，clinical efficiency and mortality of the two groups was compared.Results There were no significant difference in the hematoma cleared rate，mortality，clinical efficiency between the treatment group and the control group（P＞0.05）.The operative time in the treatment group was shorter than that in the control group，skull defect area after treatment in the treatment group was less than that in the control group，with significant difference　（P＜0.05）.Conclusion Soft-channel-assisted micro-bone window craniotomy can be used as the preferred method to treat basal ganglia hemorrhage complicated with cerebral hernia.
