腹水浓缩回输术治疗肝硬化腹水40例效果评价

摘要目的探讨腹水浓缩回输术治疗肝硬化腹水40例效果。方法选取2015年2月～2016年1月我院收治的80例肝硬化腹水患者，分为A组与B组，各40例，两组患者均予以利尿、保肝治疗，A组于传统治疗的基础上予以多次治疗性腹部穿透放液治疗，B组于传统治疗的基础上予以腹水浓缩回输术治疗，每次超滤量为4000～10 000 ml，治疗时间控制在1.5～3.0 h，1次/周，观察两组患者治疗前后的体重、腹围、24 h内尿量、其血清血蛋白（ALB）、肾小球滤过率及24 h尿钠浓度及治疗后临床疗效。结果治疗4周后，B组肝硬化腹水患者的体重为（64.87± 6.23）kg，腹围为（64.14±7.14）cm，24 h内尿量为（1487.87±401.41）ml，均优于A组，P＜0.05，ALB为（31.71±5.23）g/L，肾小球滤过率为（67.14±14.80）ml/min及24 h尿钠浓度为（72.87±10.03）mmol/L，均高于A组，P＜0.05，且肌酐（Cr）浓度为（124.51±27.13）μmol/L，胱抑素C（Cys）浓度为（1.24±0.23）mg/L，均低于 A组，P＜0.05，B组的总有效率为85.00%，高于A组，P＜0.05。结论腹水浓缩回输术治疗肝硬化腹水的效果显著,值得在临床中推广实施。

Abstract：ObjectiveTo study the treatment effect of concentration back to lose of 40 patients with cirrhotic ascites.Methods80 patients with liver cirrhosis ascites in our hospital from February 2015 to January 2016 were selected and divided into group A and group B,with 40 cases in each group.Patients of two groups were given dieresis and protect liver treatment,on the basis of conventional treatment,group A was given multiple therapeutic abdomen through dispersing treatment;on the basis of conventional treatment,group B was given the treatment of ascites concention doping,ultrafiltration volume was 4000-10 000 ml each time,treatment time control in 1.5-3.0 h,1 time/week.The liver cirrhosis ascites weight, 6,24 hour urine,blood serum protein,glomerular filtration rate and 24 hours urinary sodium concentration before and after treatment and clinical curative effect after treatment of patients in the two groups were observed.ResultsIn 4 weeks after treatment,weight of B group was(64.87±6.23)kg,abdominal girth was(64.14±7.14)cm,urine volume in 24 hours was(1487.87±401.41)ml,were superior to those of the group A(P＜0.05);the serum ALB was(31.71±5.23)g/L, glomerular filtration rate was(67.14±14.80)ml/min and 24 hours urinary sodium concerntration was(72.87±10.03)mmol/L, were higher than those of the group A(P＜0.05);Cys concention was(1.24±0.23)mg/L,was lower than those in group A (P＜0.05),the total effective rate of group B was 85.00%,higher than that of group A(P＜0.05).ConclusionThe ascites cercention doping technique for the treatment of ascites significantly is worth popularizing in clinical implementation.
