摘要目的 探讨有晶体眼人工晶体植入术治疗高度近视的临床效果。方法 选取2017年3月~2019年6月大连市第三人民医院收治的80例高度近视患者,按照随机数字表法分为观察组和对照组,每组各40例。观察组实施有晶体眼后房型人工晶体植入术(ICL),对照组实施全飞秒激光微小切口角膜基质透镜切除术(SMILE)。比较两组术后1年,3 mm 与5 mm 瞳孔直径时晶状体高阶像差及全眼高阶像差,手术前后角膜内皮细胞密度情况,随访1年期间视力变化情况。结果 术后1年,观察组3 mm 与5 mm 瞳孔直径时晶状体高阶像差水平及全眼高阶像差水平均低于对照组,差异有统计学意义(P<0.05);观察组角膜内皮细胞密度大于对照组,差异有统计学意义(P<0.05)。术后1个月、术后6个月和术后1年,观察组视力均高于对照组,差异有统计学意义(P<0.05)。结论 针对高度近视患者,实施ICL 植入术,能有效地降低术后高阶像差,改善角膜内皮细胞功能,提高术后视力。
Abstract:Objective To explore the clinical effect of intraocular lens implantation in the treatment of high myopia.Methods A total of 80 patients with high myopia admitted to Dalian Third People′s Hospital from March 2017 to June 2019 were selected,and divided into the observation group and the control group according to random number table method,with 40 patients in each group.The observation group was treated with posterior chamber intraocular lens (ICL),the control group was treated with full femtosecond laser micro-incision corneal stroma lens resection (SMILE).At 1 year after surgery,lens high-order aberrations and whole-eye high-order aberrations at 3 mm and 5 mm pupil diameters,corneal endothelial cell density before and after surgery,visual acuity changes during 1 year follow-up were compared between the two groups.Results At 1 year after operation,the higher-order aberrations of lens in observation group were lower than those in control group at 3 mm and 5 mm pupil diameters,the differences were statistically significant(P<0.05).The density of corneal endothelial cells in observation group was higher than that in control group,the difference was statistically significant (P<0.05).One month,six months and one year after operation,the visual acuity in observation group was higher than that in control group,the difference was statistically significant (P<0.05).Conclusion For patients with high myopia,ICL implantation can effectively reduce postoperative high-order aberrations,improve corneal endothelial cell function and improve postoperative visual acuity.
