Abstract：Objective To explore the effect of prenatal nipple healthcare intervention on the incidence of postpartum mastitis in primigravidas.Methods 120 primigravidas with nipple dysplasia and 120 normal primigravidas registered in Guangming New District People′s Hospital and supervised by Dongkeng Community Health Service Center from June 2012 to January 2015 were selected as A group and B group.Two groups were randomly divided into 4 sub-groups (A observation sub-group,A control sub-group,B observation sub-group and B control sub-group 60),with cases in each sub-group.Patients in A observation sub-group and B observation sub-group were received a series of targeted nipple care interventions according to different conditions,including education on prenatal nipple healthcare and the nipple healthcare guidance;while,only general nipple healthcare and cleaning was taken for patients in A control sub-group and B control sub-group.Then,the incidence rate of preterm birth,postpartum 6-month feeding success,mastitis and nipple adverse reactions as well as breast swelling of four subgroups were analyzed.Results The incidence of postpartum cracked nipple,galactostasis and mastitis of A and B observation sub-group was lower than that of A and B control sub-group,the differences were statistically significant(P＜0.05);A observation sub-group had a higher incidence of flat and inverted nipples and higher feeding success rate than A control sub-group;the incidence of postpartum mastitis in A control sub-group was 41.7%,significantly higher than that of B control sub-group (23.3%,P＜0.05);the incidence of postpartum mastitis in A observation sub-group was 10.0%,close to that in B observation sub-group(8.3%,P＞0.05);and the incidence of preterm in four subgroups also had no significant difference (P＞0.05).the incidence ofⅡ-Ⅲdegree of breast swelling in A observation sub-group and B control sub-group was lower than A control sub-group,the difference was statistically significant(P＜0.05).Conclusion Prenatal nipple healthcare intervention can prevent the incidence of postpartum mastitis for primigravidas,especially for pregnant women with abnormal nipples,which is worthy of clinical application.
