自拟温肾健脾利水汤和双氢克尿噻治疗孕妇特发性羊水过多的效果对比

摘要目的对比自拟温肾健脾利水汤和双氢克尿噻治疗孕妇特发性羊水过多的效果。方法选择2014年6月～2016年10月我院产科就诊的200例特发性羊水过多孕妇，按照随机数字表法分为对照组和治疗组，每组各100例。对照组予双氢克尿噻治疗，治疗组予我院自拟温肾健脾利水汤治疗，两组均治疗1周。观察两组治疗前后宫高、腹围、体质量、羊水指数、症状评分；比较两组母婴并发症、分娩方式及产后并发症。结果治疗组治疗后宫高、腹围、体质量、羊水指数均较治疗前明显下降，且治疗1周后治疗组各项指标较对照组明显下降，差异均有统计学意义（P＜0.05）；治疗组总有效率高于对照组，差异有统计学意义（P＜0.05）；治疗组剖宫产率及母婴并发症发生率均低于对照组，差异有统计学意义（P＜0.05）。结论自拟温肾健脾利水汤治疗孕妇特发性羊水过多效果显著，且无明显不良反应，值得临床推广。

Abstract：ObjectiveTo compare the effect of self-made kidney-warming spleen fortifying and water disinhibiting decoction and hydrochlorothiazide on treating idiopathic polyhydramnios in pregnantwomen.M ethodsFrom June 2014 to October 2016,200 pregnantwomen suffering from idiopathic polyhydramnios in Department of Obstetrics of our hospital were selected and divided into control group(n=100)and treatment group(n=100)according to the random numbers tablemethod.In the control group,hydrochlorothiazide was used,while in the treatment group,self-made kidney-warming spleen fortifying and water disinhibiting decoction was adopted.The course of treatmentwas lasted for one week in both groups.The fundal height,abdominal girth,body mass,amniotic fluid index,symptom score as well asmaternal and child complications,deliverymode and postpartum complicationswere compared between the two groups.ResultsIn the treatment group,the fundal height,abdominal girth,body mass,and amniotic fluid index were greatly decreased after therapy.The decrease of each index after one-week treatmentwasmore obvious than that in the control group with statistical differences (P＜0.05).The total effective rate in the treatment group were also higher in comparison with those in the control group,which was displayed a statistical difference(P＜0.05).The rate of cesarean section and incidence ofmaternal and child complicationswere both lower in the treatment group after comparison of those in the control group with statistical differences(P＜0.05).ConclusionSelf-made kidney-warming spleen fortifying and water disinhibiting decoction obtains a remarkable effect on treating idiopathic polyhydramnios in pregnantwomen withoutmarked adverse reactions,which isworthy of clinical promotion.
