NT-proBNP和hs-cTnT联合检测在慢性心功能不全患者预后评估中的效果

摘要目的分析N末端B型利钠肽（NT-proBNP）和高敏肌钙蛋白（hs-cTnT）联合检测评估慢性心功能不全（CCI）患者预后的效果。方法选取于2013年1月～2016年12月东莞市常平医院治疗的60例CCI患者，根据纽约心功能分级（NYHA）分级情况分为观察Ⅰ～Ⅳ组，另选择同期60例体检健康者为对照组。以免疫学检测法观察患者及健康对照组的NT-proBNP、hs-cTnT水平的变化情况。结果观察组患者NT-proBNP、hs-cTnT水平高于健康对照组，差异有统计学意义（P＜0.05）；在观察组内不同NYHA分级患者中，随着分级越高，患者NT-proBNP、hs-cTnT水平越高，组间比较具有统计学差异（P＜0.05）。Pearson分析NYHA与患者NT-proBNP、hs-cTnT水平正相关（r= 0.325,0.652，P＜0.05）。结论 NT-proBNP和hs-cTnT联合检测对于慢性心功能不全患者的临床诊断及其预后效果评估均具有一定临床应用价值，值得临床推广。

Abstract：ObjectiveTo evaluate the prognostic value of combined detection of NT-proBNP and hs-cTnT in patients with chronic cardiac insufficiency.Methods60 patients with CCI treated in Changping Hospital of Dongguan from January 2013 to December 2016 were selected as and the observation group and were divided into groupⅠ-Ⅳ according to the classification of NYHA(cardiac function classification in New York);another 60 healthy persons were selected as the control group.The changes of NT-proBNP and hs-cTnT levels were observed in patients and healthy controls during hospitalization and 12 months′follow-up.ResultsThe levels of NT-proBNP and hs-cTnT in the observation group were higher than those in the healthy control group,and the differences were statistically significant(P＜0.05).In the observation group of different NYHA classification,the higher the grade,the higher the level of NT-proBNP and hs-cTnT,the difference was statistically significant(P＜0.05).Pearsons analysis of NYHA was positively correlated with NT-proBNP and hs-cTnT(r=0.325,0.652,P＜0.05).ConclusionThe combined detection of NT-proBNP and hs-cTnT has clinical value in the clinical diagnosis and prognosis evaluation of CCI patients,which is worthy of clinical application.
