尼尔雌醇联合米索前列醇在绝经后妇女节育器取出中的应用

摘要目的探讨尼尔雌醇联合米索前列醇在绝经后妇女节育器取出中的应用效果。方法选择2014年1月～2015年12月在我院拟行宫内节育器取出的120例绝经后女性患者，随机分为观察组和对照组，各60例。对照组取环术前不用任何药物，观察组取环术前1周顿服尼尔雌醇6 mg，术前4 h阴道后穹隆放置400 μg米索前列醇片，然后进行取环手术。观察比较两组的宫颈软化程度、手术的效果以及术中患者的疼痛程度。结果观察组宫颈软化率为95.00%，明显高于对照组的75.00%，差异有统计学意义（P＜0.05）。观察组手术成功率为100.00%，明显优于对照组（81.67%），差异有统计学意义（P＜0.05）。观察组VAS评分为（1.46±0.58）分，明显低于对照组的（3.44± 0.87）分，差异有统计学意义（P＜0.05）。结论在绝经后妇女取环术中联合应用尼尔雌醇及米索前列醇，可以有效软化宫颈，提高取环成功率，患者术中疼痛程度较低，值得临床上广泛推广。

Abstract：Objective To explore the application effect of Nilestriol combined with Misoprostol in the removal of intrauterine device for postmenopausal women.Methods 120 cases of postmenopausal women with IUD removed from January 2014 to December 2015 in our hospital were selected and randomly divided into observation group and control group,60 cases in each group.The control group was not use any drugs before operation of drawing the IUD,the observation group was given the Nilestriol 6 mg one week before operation,and Misoprostol Tablets 400 μg was placed in the posterior vaginal fornix 4 hours preoperative.The degree of cervical softening,the effect of the surgery and intraoperative pain of the patients in the two groups were observed and compared.Results The cervical softening rate of the observation group was 95.00%,it was significantly higher than that of the control group（75.00%）,there was significant statistical difference（P＜0.05）.The success rate of the observation group was 100.00%,it was significantly better than that of the control group（81.67%）,the difference was statistically significant（P＜0.05）.The VAS scores of the observation group was（1.46±0.58）scores,which was significantly lower than that of the control group（3.44±0.87）scores,the difference was statistically significant（P＜0.05）.Conclusion The combined application of Nilestriol and Misoprostol in removal of intrauterine device for postmenopausal women,can effectively soften the cervix,improve the success rate of IUD removal, and the pain degree of patients is lower,it is worthy of extensive promotion.
