宫腔镜治疗子宫瘢痕憩室患者不良预后的影响因素分析

Abstract Objective To explore the influencing factors of poor prognosis of hysteroscopy in the treatment of patients with uterine scar diverticulum.Methods A total of 80 patients with uterine scar diverticulum who underwent hysteroscopic repair in Nanchang Institute of Medical Sciences from January 2018 to January 2020 were selected.The poor prognosis of patients were counted,the baseline of patients′datas were collected,the factors that may lead to poor prognosis of uterine scar diverticulum were analyzed.Results Six months after operation,65 patients had good prognosis,accounting for 81.25%,and 15 patients had poor prognosis,accounting for 18.75%.Univariate analysis showed that age,duration of menstruation,diverticulum width and myometrium thickness of adjacent uterus were compared between patients with good prognosis and those with poor prognosis,the differences were not statistically significant(P＞0.05);the uterine position,diverticulum depth,residual myometrium thickness,cesarean section times,postoperative infection,the time from the last cesarean section to operation were compared between the patients with good prognosis and those with poor prognosis,the differences were statistically significant(P＜0.05).The results of multi factors analysis found that eeper depth of the diverticulum,thinner thickness of the residual muscle layer,retroposition of uterus,postoperative infection were risk factors that affected the poor prognosis of the uterine scar diverticulum treated by hysteroscopy(OR＞1,P＜0.05).Conclusion Deeper depth of the diverticulum,thinner thickness of the residual muscle layer,retroposition of uterus,postoperative infection are the factors that affect the poor prognosis of uterine scar diverticulum treated by hysteroscopy.
