经浅表联合直肠超声在睾性及睾后性无精症诊断中的价值

Abstract Objective To investigate the diagnostic effect and value of trans-superficial combined with rectal ultrasound in patients with testis and epididymal asthenia.Methods A total of 90 patients with testis and epididymal asthenia admitted to the Department of Ultrasonography of Huichang County People′s Hospital from May 2014 to March 2020 were selected as subjects.All patients were confirmed by sediment microscopy and surgical examination (gold standard).All patients were proposed to undergo trans-superficial combined with rectal ultrasound examination before diagnosis,and the results were compared with those of sediment microscopy.The ROC curve was drawn to analyze the diagnostic efficacy (sensitivity and specificity) of trans-superficial combined with rectal ultrasound in patients with testis and epididymal asthenia.Results A total of 45 cases were confirmed by superficial ultrasound,and the diagnosis accordance rate was 50.00%,compared with the gold standard,the difference was statistically significant (P＜0.05).A total of 54 cases were confirmed by rectal ultrasound,and the diagnosis accordance rate was 60.00%,compared with the gold standard,the difference was statistically significant (P＜0.05).A total of 81 cases were finally confirmed by the combined test,and the diagnosis accordance rate was 90.00%,compared with the gold standard,there was no significant difference (P＞0.05).Trans-superficial combined with rectal ultrasound indicated 18 cases of epididymitis,9 cases of seminal vesiculitis,9 cases of ejaculatory duct cyst,9 cases of congenital small volume of testis and prostate,and 45 cases of no obvious abnormality.The ROC curve showed that the diagnostic sensitivity and specificity of trans-superficial combined with rectal ultrasound were higher than those of trans-superficial and rectal ultrasound alone (P＜0.05).Conclusion Trans-superficial combined with rectal ultra sound has a high diagnosis accordance rate in the patients with testis and epididymal asthenia,which can obtain higher diagnostic sensitivity and specificity,provide imaging basis for clinical diagnosis and treatment,and is worthy of popularization and application.
