Clinical significance of dynamic monitoring of procalcitonin on antibiotic use in patients with ICU sepsis |

Abstract Objective To explore the dynamic monitoring of procalcitonin (PCT) in guiding the use of antibiotics in patients with sepsis.Methods A total of 90 patients with sepsis admitted to our hospital from August 2016 to August 2018 were selected as subjects.According to whether the dynamic PCT monitoring was performed or not,they were divided into the conventional group (n=42) and the PCT group (n=48).All were routinely monitored and treated with a sepsis treatment regimen.The standard of the guideline was used to determine the antibiotic treatment plan in the control group.The PCT group was monitored by PCT on the 1st,3rd,5th,7th,and 9th days after admission,and the antibiotic application program was considered comprehensively.The time and cost in the intensive care unit (ICU),antibacterial use time,and mortality were compared between the two groups.According to the patient outcome,the PCT group was further divided into the death group (n=8) and the survival group (n=40).The PCT value,sequential organ failure assessment (SOFA) score,acute physiology and chronic health (APACHE Ⅱ) score between the death group and the survival group were compared.Results There was no significant difference in death rate between the conventional group and the PCT group (P>0.05).The ICU stay timeand antibiotic use time in the PCT group were shorter than those in the conventional group,the ICU hospitalization cost of the PCT group was lower than that of the conventional group,the differences were statistically significant (P<0.05).The SOFA score,APACHE Ⅱscore and PCT levels on the 1st,3rd,5th,7th and 9th days after admission of PCT survival group were lower than those of the death group,the differences were statistically significant (P<0.05).Conclusion PCT dynamic monitoring can not only judge whether patients have bacterial infection,the degree of infection and prognosis of patients,but also guide clinical rational drug use,reduce ICU hospital stay as well as cost,and has good clinical value,which is worthy of promotion.
