Clinical effect of intratracheal instillation of Bovine Pulmonary Surfactant for Injection combined with Budesonide in the treatment of neonatal respiratory distress syndrome |

Abstract Objective To study the clinical effect of intratracheal instillation of Bovine Pulmonary Surfactant for Injection combined with Budesonide in the treatment of neonatal respiratory distress syndrome (NRDS). Methods Sixty children with NRDS admitted to the Department of Neonatology of Xinyu People′s Hospital from May 2017 to May 2019 were selected as subjects. They were divided into control group (n=30) and observation group (n=30) by lottery. The control group was treated with Bovine Pulmonary Surfactant for Injection, while the observation group was treated with intratracheal instillation of Bovine Pulmonary Surfactant for Injection combined with Budesonide. The blood gas analysis, clinical manifestations, oxygen saturation (SpO2), heart rate, invasive mechanical ventilation rate, mechanical ventilation time, hospital stay, biparietal diameter (BPD) incidence and total complication rate were observed. Results After treatment, arterial oxygen partial pressure (PaO2) and pH value of the observation group were higher than those of the control group, and mechanical blood carbon dioxide partial pressure ( PaCO2) was lower than that of the control group,the differences were statistically significant (P<0.05). The scores of clinical manifestations including polypnea, groan,three concave signs, and complexion in the observation group were lower than those of the control group, the differences were statistically significant (P<0.05). The SpO2 in the observation group was higher than that in the control group, and the heart rate was lower than that in the control group, the differences were statistically significant (P<0.05).In the observation group, the invasive mechanical ventilation rate was lower, the mechanical ventilation time and hospital stay were shorter, and the incidence of BPD and the total incidence of complications were lower compared with those in the control group, the differences were statistically significant (P<0.05). Conclusion Intratracheal instillation of Bovine Pulmonary Surfactant for Injection combined with Budesonide for children with NRDS can effectively improve blood gas analysis and clinical symptoms, reduce the invasive mechanical ventilation rate, the incidence of BPD and complications, and shorten mechanical ventilation time and hospital stay, which is worthy of promotion.
