[Abstract]Objective To analyze the unreasonable medical orders of pharmacy intravenous admixture service(PIVAS),to understand the unreasonable medical orders,and to formulate the"matters needing attention for clinical intravenous drug use"in our hospital,so as to ensure the safety,rationality and effectiveness of clinical drug use.Methods in September,2018,unreasonable doctors′orders were selected from the venous drug dispensing center of our hospital in 12 months of 2017 and unreasonable doctors′orders were collected and analyzed 8 months after the formulation of notes for common clinical intravenous drug use.Results In 2017,there were 2254 unreasonable medical orders,accounting for 0.53%.A total of 118 unreasonable medical orders,accounting for 0.04%,were formulated after"matters needing attention for common clinical intravenous drug use".The main reasons for unreasonable doctors′advice were unreasonable selection of solvent and unreasonable dosage of solvent.Conclusion Designed to department as the unit of the clinical commonly used intravenously matters needing attention,issued to the clinical departments,to guide the clinical medication of normative,unreasonable medical advice rate dropped significantly,can be used as PIVAS pharmacist prescription audit evaluation criteria,to live from physicians,provide reference for new nurses,in the specification the doctors′advice and the rational use of drugs has played a very good guide.
