医院-社区-家庭一体化管理对慢性阻塞性肺疾病高龄患者生命质量的影响

Abstract ]Objective To explore the effect of Hospital-Community-Family integrated management on the quality of life of elderly patients with chronic obstructive pulmonary disease.Methods A total of 80 elderly patients with chronic obstructive pulmonary disease(COPD)admitted to our center and Jianshe Community Health Service Center From May to December 2016 were selected as the study subjects,they were divided into control group and research group by random number table method,with 40 cases in each group.The control group was given routine nursing intervention,while the research group was given Hospital-Community-Family integrated management on the basis of the control group.Blood gas index,lung function index and quality of life score were compared between the two groups.Results The arterial partial pressure of oxygen (PaO2)after implementation was higher than before,and the partial pressure of carbon dioxide(PaCO2)and pH were lower than before,the differences were statistically significant(P＜0.05).The PaO2of the study group was higher than that of the control group,and the PaCO2and pH of the study group were lower than that of the control group,the differences were statistically significant(P＜0.05).The forced expiratory volume(FEV1),forced vital capacity (FVC),the ratio of FEV1to FVC (FEV1%),peak expiratory flow (PEF)and FEV1/FVC were significantly higher in the two groups than before,the differences were statistically significant(P＜0.05).The pulmonary function indexes in the study group were higher than those in the control group,the differences were statistically significant(P＜0.05).The total average score of quality of life(TMS)and each dimension score of the two groups after implementation were lower than those before implementation,the differences were statistically significant(P＜0.05).The total average score of quality of life (TMS)and the scores of each dimension in the study group were lower than those in the control group,the differences were statistically significant(P＜0.05).Conclusion The application of Hospital-Community-Family integrated management in elderly patients with chronic obstructive pulmonary disease can effectively improve pulmonary function and blood gas index,and improve the quality of life of patients with high clinical value.
