老年高血压患者应用社区健康管理的效果

Abstract Objective To explore the clinical effect of community health management in elderly patients with hypertension.Methods From May 2015 to April 2017,300 elderly patients with hypertension treated in physical examination center,internal medicine clinic and inpatient department in Xinhua Community Health Service Center in Huadu District in Guangzhou were selected and they were divided into the study group and the control group,and there were 150 cases in each group according to the random number method.Patients in control group were given the routine care intervention,while the study group was given community health management on the basis of the control group.The blood pressure,body mass index,fasting blood glucose,triglyceride,cholesterol index,health behavior awareness,healthy lifestyle,anxiety and depression score,SF-36 score,incidence of rehospitalization and the incidence of adverse reactions were compared between the two groups.Results After the intervention,the blood pressure,body mass index,fasting blood glucose,triglyceride and cholesterol index of the study group were significantly improved compared with the control group (P<0.05).The proportion of healthy lifestyles in the study group was significantly higher than that in the control group (P<0.05).The score of anxiety and depression in the study group were significantly lower than those in the control group (P<0.05).The awareness rate of health behavior and SF-36 score in the study group were significantly higher than those in the control group (P<0.05).The incidence of hospital admission and the incidence of adverse reactions were significantly lower in the study group than in the control group (P<0.05).Conclusion Community health management applied in elderly hypertensive patients can improve the anxiety and depression of patients,improve the life quality of patients,control the blood pressure level of patients,reduce the incidence of adverse reactions and hospital readmission,and it is worthy of clinical promotion and application in clinic.
