预注不同血管活性药对重度子痫前期患者腰硬联合麻醉剖宫产母儿的影响

Abstract Objective To study the influence of preoperative injection of different vasoactive drugs on the maternal and neonatal during the spinal-epidural anesthesia of cesarean section in severe pre-eclampsia patients.Methods From December 2016 to December 2017,60 severe pre-eclampsia patients undergoing the spinal-epidural anesthesia in our hospital were selected as the subjects,and divided to three groups by radom digital table method,20 cases in each group.Group A took the pumping injection of ephedrine (2 mg/min).Group B took the pumping injection of norepinephrine(25 ug/min).Group C took the equivalent normal saline.The final outcomes were observed.Results At 1,10,12 and 15 min,there was no significant difference in the blood pressure among three groups(P＞0.05).At 3,5 and 7 min,the blood pressure in Group C was significantly lower than that in Group A and Group B (P＜0.05).At 1 min,there was no significant difference in the heart rate among three groups(P＞0.05).At 3,5,7,10,12 and 15 min,the heart rate in Group C was significantly higher than that in Group A and Group B,the difference was statistically significant(P＜0.05).After 5 min,the heart rate in Group B was significantly higher than that in Group A,the difference was statistically significant(P＜0.05).There were no significant a difference in the lactic acid,PCO2,PO2,BE and pH among three groups (P＞0.05).The incidence rate of nausea and vomiting in Group A and Group B was lower than that in Group C,the difference was statistically significant(P＜0.05).Conclusion During the spinal-epidural anesthesia of cesarean section,the preoperative injection of Ephedrine and Norepinephrine can maintain the hemodynamics stability,keep the blood concentration and improve the maternal and neonatal prognosis.
