2型糖尿病合并亚临床甲状腺功能减退症116例临床分析

Abstract Objective To observe the clinical feature of patients with type 2 diabetes complicated with subclinical hypothyroidism　（SCH）and to analyze its possible effect. Methods The clinical data of 840 patients with type 2 diabetes hospitalized in the department of endocrinology of Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from January 1,2014 to October 31 were selected and retrospectively analyzed.Among 840 patients,those combined with SCH were divided into observation group（group A）and the other patients into control group（group B）. The age,course of disease,gender ratio,thyroid function,TgAb,TpoAb,blood lipid profile,and complications of diabetes in patients between two groups was compared respectively. Results Compared with group B,the female ratio（60.3％vs 49.2％）,TSH level [（6.44±4.57）mU/L vs（1.82±0.83）mU/L],TgAb level [（124.99±302.37）U/ml vs（23.19±80.47）U/ml], TpoAb level [（125.79±355.14）U/ml vs（20.95±108.85）U/ml],total CHOL level [（5.30±1.46）mmol/L vs（4.90±1.24）mmol/L], LDL-C level [（3.27±1.10）mmol/L vs（3.06±0.99）mmol/L] was higher and the incidence rate of combined CHD（39.7％vs 26.8％）,low extremity atherosclemsis disease（54.3％vs 43.4％）,diabetic retinopathy（DR）（28.4％vs 20.4％）,diabetic kidney disease（DKD）（31.9％vs 23.1％）and diabetic peripheral neuropathy（DPN）（50.0％vs 39.9％）was higher that of group A,and the level of FT4was lower [（14.37±3.27）pmol/L vs（15.74±2.98）pmol/L].Except for DR（P＝0.052）,the difference of above-mentioned index was all statistical （P＜0.05）. Conclusion Combined SCH may have an adverse effect on type 2 diabetic patients.Therefore,patients with type 2 diabetes,especially females,patients with higher thyroid antibody levels,patients with abnor-book=30,ebook=36mal blood lipid metabolism are suggested to screen thyroid function regularly. For patients with type 2 diabetes complicated with SCH,the incidence rate of complication of diabetes is higher and more attention should be paid on disease condition to reduce related risk.
