放射性131I治疗分化型甲状腺癌的临床效果及对患者唾液腺功能的影响

Abstract Objective To evaluate and analyze the clinical effect of radioactive iodine131I in the treatment of differentiated thyroid carcinoma and study the effect of radioactive iodine131Ion the function of salivary gland.M ethods 86 patientswith differentiated thyroid carcinomawere selected as the object of study and treated in our hospital from January 2012 to January 2016.The size of residual thyroid tissue and the function of salivary gland weremeasured by salivary gland dynamic imaging before treatment.Effect of removing the residual thyroid gland were evaluated by131Iwhole body imaging after 3-6 months treatment.Themaximum accumulation rate　（MAR）and maximum secretion rate　（MSR）of parotid and submandibular gland before using131Iand at 10 days，1 month and 3 months after using131Iweremeasured by salivary gland scintigraphy.Results Among 86 cases，61 caseswere completely removed，accounting for 70.93%.25 cases were not completely removed，accounting for 29.07%.42 out of 95 metastatic lesions were cured，accounting for 44.21%，32 were effective，accounting for 33.68%，and 21 were invalid，accounting for 22.11%.MAR and MSR of parotid gland were significantly decreased at 10 days and 1 month after using131I compared to those before using131I　（P＜0.05）.At 3 months after using131I，MAR and MSR of parotid gland returned to normal level.MAR of submandibular gland decreased significantly　at 10 days after using131I　（P＜0.05），MSR of submandibular gland decreased significantly at 1 month after using131I（P＜0.05）.The patients had no any serious functional impairment of salivary gland after treatment.Conclusion The application of radioactive iodine131I is a safe and effectivemethod applicating in the treatment of differentiated thyroid carcinoma for the removal of residual thyroid tissue.Radioactive iodine131I therapy may lead to damage of salivary gland function，but ithas reversibility，which would recoverwithin 3months.
