AIMS65评分系统对上消化道出血评估作用的临床价值研究

Abstract ObjectiveTo explore the clinical predictive value of AIMS65 scoring system in upper gastrointestinal hemorrhage through retrospective analysis its mortality,rate of blood transfusion,rebleeding rate.MethodsClinical data of 567 cases of upper gastrointestinal hemorrhage admitted into our hospital from January 2011 to December 2015 were retrospectively analyzed.According to the score (0 to 5 points),which were divided into 6 groups,mortality,rate of blood transfusion,rebleeding rate among groupswere compared.The ROC curve was used to calculate the area under the curve (AUC)to evaluate the value of the scoring system in predicting the death,blood transfusion and rebleeding of upper gastrointestinal hemorrhage.ResultsThe mortality of 1-6 groups was 0.0%,2.9%,12.7%,29.2%,42.9%and 94.4%respectively;rate of blood transfusion rate was 9.3%,17.1%,27.1%,44.1%,50.0%,72.2%respectively;rebleeding rate was 3.3%,6.4%,5.4%,11.8%,13.3%,22.2%respectively.The AUC ofmortality,rate of blood transfusion,rebleeding rate was 0.888,0.723 and 0.649 respectively.When AUC≥0.7,scoring system had good diagnostic value.ConclusionAIMS65 scoring system for upper gastrointestinal bleeding mortality,blood transfusion rate has a good diagnostic value,the rebleeding rate of diagnosis is low,the score is simple,easy to clinical collection,has a good clinical value.
