纳布啡复合罗哌卡因在痔疮手术后镇痛的应用

Abstract ObjectiveTo explore the safety and effect of Nalbuphine combined with Ropivacaine for postoperative analgesia of hemorrhoid.Methods120 patients with severe hemorrhoids undergoing anorectal surgery from September 2016 to March 2017 in our hospital were selected,the ASA classification was fromⅠtoⅡgrade.All patients were divided into group A,B and C according to the random number table method,40 cases in each group.All patients were given postoperative epidural self-controlled analgesia.Group A was given 0.5 μg/ml Sufentanil combined with 0.15%Ropivacaine, group B was given 0.1 mg/ml Nalbuphine combined with 0.15%Ropivacaine,group C was given 0.2 mg/ml Nalbuphine combined with 0.15%Ropivacaine.The VAS scores at postoperative 8,12,24,48 h and at the first defecation in the each group were recorded,while the incidence of adverse reactions such as anal edema,pruritus,nausea,vomiting,and difficulty in breathing in each group were observed.Results The VAS scores of group A and group C at 12,24 and 48 h were lower than those in group B,with significant difference(P＜0.05).The VAS score at the first defecation of group C was lower than that of group A and group B,with significant difference (P＜0.01).The incidence rate of pruritus,nausea and vomiting in group B and group C was lower than that in group A,with significant difference(P＜0.05).Conclusion0.1 mg/ml or 0.2 mg/ml Nalbuphine combined with 0.15%Ropivacaine is safe and effective for postoperative analgesia of hemorrhoid,and the incidence rate of itching and nausea and vomiting is lower than that of 0.5 ug/ml Sufentanil combined with 0.15%Ropivacaine.
