老年糖尿病患者护理中应用个案管理护理模式的可行性

Abstract ObjectiveTo explore the feasibility of case management nursing model in elderly patients with diabetes mellitus.Methods138 patients with diabetes mellitus in our hospital from January 2014 to January 2016 were selected and divided into the observation group and the control group,with 69 cases in each group.The control group was given routine diabetes care mode,while the observation group was given case management model.The levels of FBG,HbA1c,diabetes knowledge,quality of life and self behavior management were compared between two groups.ResultsBefore intervention,there were no significantly different among the levels of FBG,HbA1c in two groups(P＞0.05);after intervention,the levels of FBG,HbA1c in two groups were all decreased,and compared with those before nursing,the differences were statistically significant(P＜0.05).And those in the observation group was lower than those in the control group,and the differences were statistically significant(P＜0.05).Before intervention,there were no significantly different among the scores of DKT,DSQL self behavior management(diet control,exercise management,foot care,drug monitoring,hyperglycemia processing)in two groups(P＞0.05);after intervention,the scores of DKT and all indexes in self behavior management in the observation group were increased,the score of DSQL in the observation group were decreased,and compared with those before nursing,the differences were statistically significant(P＜0.05),but there were no statistically significant differences in the control group between those before and after intervention(P＞0.05).And the scores of DKT and all indexes in self behavior management in the observation group was higher than those in the control group,the scores of DSQL was lower than that in the control group,and the differences were statistically significant(P＜0.05).ConclusionThe application of case management model in elderly patients with diabetes mellitus can effectively control the blood glucose and improve the self behavior management ability of diabetes in patients.It is worthy to be popularized.
