新型助力哺乳枕辅助侧卧式哺乳对剖宫产产妇纯母乳喂养率及母乳喂养效能的影响

摘要目的探讨新型助力哺乳枕辅助侧卧式哺乳对剖宫产产妇纯母乳喂养率及母乳喂养效能的影响。方法选取2019 年1 月至2020 年10 月来自宜春市妇幼保健院的105 例剖宫产产妇作为研究对象，采用随机抽签法分为对照组（52 例）及观察组（53 例），对照组采用无干预侧卧式哺乳，观察组采用新型助力哺乳枕辅助哺乳。比较两组患者产后第1、42 天喂养方式（纯母乳、纯人工、混合喂养）、母乳喂养效能[母乳喂养自我效能量表（BSES）]及母乳喂养并发症（乳头皲裂、乳房胀痛）发生情况差异。结果观察组产后第42 天的纯母乳喂养率高于对照组，差异有统计学意义（P＜0.05）。观察组产后第42 天的BSES 评分高于产后第1 天，且观察组产后第42 天的BSES 评分高于对照组，差异有统计学意义（P＜0.05）。观察组产后第42 天的母乳喂养并发症总发生率低于对照组，差异有统计学意义（P＜0.05）。结论采用新型助力哺乳枕辅助侧卧式哺乳可有效增强剖宫产产妇喂养行为，提高纯母乳喂养率，改善其母乳喂养效能，并防止乳房胀痛、乳头皲裂等并发症发生。

Abstract：Objective To explore the effect of new assisted breastfeeding pillow supplemented with breastfeeding in latericumbent posture on exclusive breastfeeding rate and breastfeeding efficiency of puerperae with cesarean section.Methods A total of 105 women who underwent cesarean seetion in Yichun Maternal and Child Health Hospital were selected from January 2019 to October 2020, and they were divided into control group (n=52) and observation group(n=53) by the lottery method. The control group were given breastfeeding in latericumbent posture without intervention,and the observation group applied new assisted breastfeeding pillow supplemented with breastfeeding in latericumbent posture. On the 1 and the 42 days after delivery, the feeding methods (exclusive breastfeeding, pure artificial feeding,mixed feeding), breastfeeding efficiency (breastfeeding self-efficacy scale [BSES]) and the occurrence of breastfeeding complications (chapped nipples, breast distending pain) of the two groups were compared. Results The rate of exclusive breastfeeding on the 42 days after delivery in the observation group was higher than that in the control group, and the difference was statistically significant (P<0.05). The BSES score on the 42 days postpartum of the observation group was higher than that on the 1 day postpartum, and the BSES score of the observation group on the 42 days postpartum was higher than that of the control group, the difference was statistically significant (P<0.05). The total incidence of breastfeeding complications on the 42 day postpartum in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). Conclusion New assisted breastfeeding pillow supplemented with breastfeeding in latericumbent posture can effectively enhance the exclusive breastfeeding behaviors, increase the exclusive breastfeeding rate, improve the breastfeeding performance, and prevent the occurrence of complications such as breast distending pain and chapped nipples in puerperae undergoing cesarean section.
