Effect comparison of Parecoxib Sodium and Dexmedetomidine alone and combined use on postoperative early cognitive function in elderly patients |

Abstract ObjectiveTo study and compare the effect of Parecoxib Sodium and Dexmedetomidine alone and combined use on postoperative early cognitive function in elderly patients.MethodsFrom July 2016 to June 2017,137 elderly patients who underwent total hip replacement,femoral head replacement and total knee joint replacement in our hospital were selected as subjects.According to the randomized control method,the patients were divided into Parecoxib Sodium group(hereinafter referred to as group P,n=42),Dexmedetomidine group(hereinafter referred to as group D,n=47),and combined group (n=48).In the operation,the treatment was performed with Parecoxib Sodium,Dexmedetomidine and a mixture of both.The concentrations of S-100β protein,neuron-specific enolase(NSE)and MoCA scores of 1 h(t0)before surgery,3 h(t1),24 h(t2)and 72 h(t3)after the surgery and the incidence rate of postoperative cognitive impairment of patients in the three groups were compared.ResultsThe serum S-100β protein content at t1of the patients in the combined group was significantly lower than those in group P and group D,the serum S-100β protein and NSE content in t2and t3were significantly lower than those in group P and group D,and the differences were statistically significant(P<0.05).The MoCA scores of the patients in the combined group were significantly higher than those in group P and group D,and the differences were statistically significant(P<0.05).The incidence rate of POCD in the group(14.5%)was significantly lower than that in group P(28.5%)and group D(29.7%),and the differences were statistically significant(P<0.05).ConclusionThe combination of Parecoxib Sodium and Dexmedetomidine in the elderly patients for maintenance anesthesia can significantly reduce the incidence of postoperative cognitive impairment.The effect is better than single use,which is worthy of promotion and application.
