Effect on diabetes mellitus and cardio-cerebrovascular events by comprehensive intervention in the community people of pre-diabetes |

Abstract Objective To investigate the effect on diabetes mellitus and cardio-cerebrovascular events by comprehensive intervention in the community people of pre-diabetes.Methods In October 2013,people diagnosed with pre-diabetes in the six communities(randomly selected from the 12 communities that had been surveyed for chronic disease control)of community health service center in our hospital were selected as subjects,a total of 712 cases.Three communities(366 cases)in the six were randomly selected as the enhanced intervention group,and the other three communities(346 cases)were selected as the control group.The intervention group was treated with enhanced intervention,and the control group was treated with routine treatment.The fasting blood glucose,postprandial blood glucose,triacylglycerol,cholesterol,waist circumference,hip circumference and blood pressure were compared before and after the intervention of the two groups,and the incidence of cardio-cerebrovascular disease of the two groups were compared.Results Before intervention,the waist circumference,hip circumference,blood glucose,systolic blood pressure,cholesterol and triacylglycerol were compared in the two groups,the differences were not statistically significant(P>0.05).After intervention,the waist circumference,fasting blood glucose,postprandial blood glucose,cholesterol and systolic blood pressure of the enhanced intervention group were significantly lower than those of before intervention,the differences were statistically significant(P<0.05).The fasting blood glucose,postprandial blood glucose and systolic blood pressure of the control group were significantly higher than those of before intervention,the differences were statistically significant(P<0.05).After intervention,the waist circumference,fasting blood glucose,postprandial blood glucose,systolic blood pressure,cholesterol and triacylglycerol of the enhanced intervention group were significantly lower than those in the control group,the differences were statistically significant(P<0.05).The incidence of progress from pre-diabetes to diabetes of the enhanced intervention group (9.02%)was significantly lower than that in the control group(16.47%),the difference was statistically significant(χ2=10.73,P<0.01).The total incidence of cardio-cerebrovascular events of the enhanced intervention group (3.28%)was significantly lower than that in the control group (7.80%),the difference was statistically significant(χ2=6.33,P=0.012).Conclusion Targeted community comprehensive intervention is more effective to prevent or delay of progress from pre-diabetes to diabetes than conventional treatment,and to reduce the incidence of cardio-cerebrovascular events in the people of pre-diabetes,too.So it can provide a new strategy for the prevention of cardio-cerebrovascular diseases.
