Analysis of the significance and value of procalcitonin in the early diagnosis and prognosis prediction of critical patients complicated with sepsis |

Abstract Objective To investigate the significance and value of procalcitonin in the early diagnosis and prognosis prediction of critical patients complicated with sepsis.Methods A total of 90 patients with systemic inflammatory response syndrome admitted to Liaoning Health Industry Group Fukuang General Hospital from November 2018 to February 2020 were selected.According to their clinical symptoms,the patients were divided into sepsis group (45 cases) and non-sepsis group (45 cases).Among the sepsis patients,35 patients survived and were set as the survival group.Ten patients died and were assigned to the death group.The levels of procalcitonin (PCT),C-reactive protein(CRP) and white blood cells (WBC),acute physiology and chronic health (APACHE Ⅱ) score in sepsis group and nonsepsis group,death group and survival group were compared.Results There were no significant differences in WBC count and CRP level between the sepsis group and the non-sepsis group on the first and third day of admission (P>0.05).The PCT level and APACHE Ⅱscore of sepsis group were higher than those of the non-sepsis group on the first and third day of admission,the differences were statistically significant (P<0.05).The PCT level and APACHE Ⅱscore of two groups on the third day of admission were lower than those on the first day of admission,the differences were statistically significant (P<0.05).There were no significant differences in WBC count and CRP level between the death group and the survival group (P>0.05).The PCT level,APACHE Ⅱscore of death group were higher than those of the survival group,the differences were statistically significant (P<0.05).The area under curve (AUC) and 95% CI of PCT levels to predict patients′ death were higher than those of the APACHE Ⅱscore,and the critical value is lower than the APACHE Ⅱscore,the differences were statistically significant (P<0.05).Conclusion The use of procalcitonin in the early diagnosis and prognosis of critical patients with sepsis has high accuracy and high clinical application value.
