Effect of body surface electrocardiogram on location of infarcted artery in patients with acute inferior wall myocardial infarction |

Abstract Objective To analyze the role of surface electrocardiogram in the location of infarcted artery in patients with acute inferior myocardial infarction, in order to provide basis for the optimization of the diagnosis and treatment of patients with acute inferior myocardial infarction. Methods A total of 50 patients with acute inferior myocardial infarction treated in Yichun Hospital of Traditional Chinese Medicine from June 2019 to May 2020 were selected as the research subjects, all selected patients underwent surface electrocardiogram and coronary angiography (CAG),the results of CAG were used as the"gold standard"to analyze the evaluation value of each parameter index of surface electrocardiogram in the location of infarcted artery in patients with acute inferior myocardial infarction. Results Among 50 patients with acute inferior myocardial infarction confirmed by CAG, 39 cases had right coronary artery infarction, accounting for 78.00%, 11 cases had left circumflex branch, accounting for 22.00%. The results of Kappa confirmed that there was a good consistency between the results of electrocardiogram and CAG in the location of infarcted artery in patients with acute inferior myocardial infarctio (Kappa=0.781, P<0.05). The surface electrocardiographic findings of left circumflex artery infarction: STaVR↓≥0.05 mV, STV3↓/STⅢ↑>1.2, electrocardiographic findings of right coronary artery: STⅢ↑/STⅡ↑>1, STaVL↓/STⅠ↓>1, STV1↑. The detection rate of STaVR↓≥0.05 mV to left circumflex coronary artery was 90.91%, the detection rate of STV3↓/STⅢ↑>1.2 was 81.82%, the detection rates of STⅢ↑/STⅡ↑>1,STaVL↓/STⅠ↓>1, STV1↑ to right coronary artery infarction were 94.87%, 92.31% and 92.31%, respectively. The detection rates of STⅢ/STⅡ↑>1, STaVL↓/STⅠ↓>1 and STV1↑ were 88.00%, 84.00% and 92.00%, respectively.Conclusion Surface electrocardiogram has high diagnostic efficiency in locating infarct artery in acute inferior myocardial infarction, clinically, the parameters of surface electrocardiogram can be used to assist in locating the infarcted artery, which can provide the basis for the formulation of early treatment regimen.
