Meta-analysis of diagnostic value of glycosylated serum albumin in diabetes mellitus |

Abstract Objective To evaluate the diagnostic value of glycated serum albumin in diabetes mellitus by meta-analysis.Methods Databases including PubMed,Cochrane Library,Wanfang,VIP,CNKI were retrieved by computer,and the literatures about the diagnostic value of glycated serum albumin for diabetes mellitus published before December 30,2020 were screened out.Diagnostic test accuracy quality assessment tool QUADAS-2 was used to evaluate the quality of the included literature,meta-disc 1.4 and Stata 15.1 software lines were used for comprehensive quantitative evaluation.Results A total of 18 articles were included.Meta-analysis showed that the combined sensitivity(SE)of glycated serum albumin in the diagnosis of diabetes mellitus was 0.73,95%CI=0.71-0.74;combined specificity (SP)was 0.89,95%CI=0.88-0.89;combined diagnostic odds ratio(DOR)was 30.52,95%CI=18.31-50.87;area under the SROC curve(AUCSROC)was 0.8366,SEAUC=0.0522.They were divided into three subgroups according to population,race and diagnostic criteria.The combined SE,combined SP,combined DOR,combined positive likelihood ratio(PLR),AUCSROCin hospital group were higher than those in community group,while the combined negative likelihood ratio (NLR)was lower than that in community group.The combined SE,combined SP,combined DOR,combined PLR and AUCSROCof the yellow race group were higher than those of the non-yellow race group,while the combined NLR was lower than that of the non-yellow race group.The combined SP,combined PLR,combined NLR in the oral glucose tolerance test(OGTT)group were higher than those in the non-OGTT group,combined SE,combined DOR and AUCSROCwere lower than non-OGTT group.Conclusion The sensitivity of glycated serum albumin in diagnosing diabetes mellitus is fair,with high specificity and high diagnostic accuracy.
