Effect of different dialysis vascular pathways on left ventricular hypertrophy and function in elderly patients with maintenance hemodialysis |

Abstract Objective To investigate the effects of different dialysis pathways on left ventricular hypertrophy and function in elderly patients with maintenance hemodialysis.Methods A total of 60 elderly patients with maintenance hemodialysis admitted to our hospital from January 2016 to January 2018 were selected as the study objects, they were divided into control group and study group according to the random number table method, with 30 cases in each group.The control group received the indwelling method of deep venous catheter, and the study group received the indwelling method of arteriovenous fistula.The cardiac structure-related indexes before and after the establishment of the two groups were compared, the cardiac function indexes and the occurrence of adverse reactions 1 year after the establishment of the two groups were compared.Results After the establishment of the pathway, the left ventricular end diastolic diameter ([52.60±4.55]mm), the inter-ventricular septum end-diastolic thickness ([12.15±1.25]mm), and the left ventricular posterior wall end-diastolic thickness ([11.50±1.65]mm) in the study group, which were higher than those of the control group ([49.80±5.30], [11.05±2.65], and [10.45±2.05]mm), the differences were statistically significant (P<0.05).After the establishment of the pathway, there were no statistically significant differences in the ventricular ejection fraction, the maximum velocity ratio of left atrial valve orifice in early and late diastolic period between the two groups (P>0.05).After the establishment of the pathway, the incidence of left ventricular hypertrophy in the study group was 60.00%, the incidence of left ventricular systolic dysfunction was 16.67%, the incidence of left ventricular diastolic dysfunction was 53.33%, and the total incidence of adverse reactions was 3.33%, all of which were lower than 83.33%,43.33%, 80.00%, and 20.00% in the control group, with statistically significant differences (P<0.05).Conclusion Compared with deep venous catheter indwelling, arteriovenous fistula indwelling has lower effect on left ventricular hypertrophy and function in elderly maintenance hemodialysis patients, and the incidence of adverse reactions is low, so it has important clinical application value.
