Clinical effect of pulmonary surfactant combined with high-frequency oscillatory ventilation and Milrinone in the treatment of persistent pulmonary hypertension of newborn |

Abstract ObjectiveTo observe the clinical effect of pulmonary surfactant combined with high-frequency oscillatory ventilationandMilrinoneinthetreatmentofpersistentpulmonaryhypertensionofnewborn(PPHN).MethodsFromMarch2015 to August 2016,114 PPHN cases treated in our hospital were enrolled as research objects and evenly divided into control group 1,control group 2,and observation group in random,38 cases in each group.Only high frequency oscillatory ventilation was used in the control group 1,and combination of high frequency oscillatory ventilation and Milrinone was applied in the control group 2,while in the observation group,high frequency oscillatory ventilation combined with pulmonary surfactant and Milrinone was adopted.The therapeutic effect,pulmonary artery systolic pressure(PASP)and mechanical ventilation time among three groups were comparatively analyzed.Results The total therapeutic effectiveness rate was 26.31%(10/38)in the control group 1,47.37%(18/38)in the control group 2,and 92.11%(35/38)in the observation group.Obviously,the total therapeutic effectiveness rate in the observation group was higher than that in the other two groups.The indexes of PASP,oxygen-intake concentration,and arterial partial pressure of carbon dioxide(PaCO2)in the observation group were much lower than those in the control groups 1 and 2 (P<0.05).Meanwhile,compared with the control groups 1 and 2,arterial oxygen pressure (PaO2)was higher,with statistical significance (P<0.05).The total mechanical ventilation time was(83.46±9.91)h in the observation group,much shorter compared with the control groups 1 and 2,[(136.35±11.23)h and(118.35±12.57)h]respectively,which was displayed a statistical difference(P<0.05).Conclusion Combination of high-frequency oscillatory ventilation,pulmonary surfactant,and Milrinone on treating PPHN obtains a great effect by improving the total effectiveness rate and PASP and shortening the mechanical ventilation,which is worthy of application and promotion.
