摘要目的系统评价物理治疗对轻度创伤性脑损伤（mTBI）后患者眩晕和平衡状态的影响。方法计算机检索PubMed、Embase、Cochrane Library、EBSCO、Web of Science、Pedro、CNKI、万方、VIP 数据库，查找关于物理治疗mTBI的相关临床研究，对纳入的研究逐个进行质量评价。并进一步对治疗后两组患者的眩晕残障程度评定量表（DHI）、Berg 平衡量表（BBS）、功能性步态评价（FGA）、活动平衡信心量表（ABC）、步行速度（Gait speed）项目的评分进行Meta分析。结果 10项研究299例患者涉及本研究系统评价。结果显示，治疗组患者的DHI 低于对照组，差异有统计学意义（P＜0.01）；BBS 高于对照组，差异有统计学意义（P＜0.01）；FGA 高于对照组，差异有统计学意义（P＜0.01）；ABC 水平高于对照组，差异有统计学意义（P＜0.01）；Gait speed 水平高于对照组，差异有统计学意义（P＜0.01）。结论物理治疗可在不同程度上改善mTBI患者创伤后的眩晕及平衡状态，促进患者早日回归工作和生活，从而提高患者的生活质量。

Abstract：Objective To systematically evaluate the effect of physical therapy on dizziness and balance of patients after mild traumatic brain injury (mTBI).Methods PubMed,Embase,Cochrane Library,EBSCO,Web of Science,Pedro,CNKI,Wanfang,VIP databases were searched to find relevant clinical studies on physical treatment of mTBI.The quality of the included studies was evaluated one by one.And further the scores of the dizziness handicap inventory(DHI),Berg balance scale (BBS),functional Gait assessment (FGA),activities balance confidence scale (ABC) and Gait speed items of the two groups were Meta-analyzed.Results A total of 299 patients in 10 studies were involved in this systematic review.The results showed that the DHI of the treatment group was lower than that of the control group,the difference was statistically significant (P＜0.01);the BBS was higher than that of the control group,the difference was statistically significant (P＜0.01);the FGA was higher than that of the control group,the difference was statistically significant (P＜0.01);ABC level was higher than that of the control group,the difference was statistically significant (P＜0.01);Gait speed level was higher than that of the control group,the difference was statistically significant (P＜0.01).Conclusion Physical therapy can improve the dizziness and balance state of mTBI patients,and promote the patients to return to work and life as soon as possible,thereby improving the patients′ quality of life.
