阴道三维超声引导下阴式手术与宫腹腔镜联合手术治疗剖宫产术后切口瘢痕憩室的效果对比

摘要目的比较阴道三维超声引导下阴式手术与宫腹腔镜联合手术治疗剖宫产术后切口瘢痕憩室的临床效果，为临床选择治疗方式提供参考。方法选取2012年6月～2016年12月在白云区人民医院行剖宫产术后切口瘢痕憩室修补术的46例患者作为研究对象，按照手术方式的不同分为阴道三维超声引导下阴式手术组（38例）和宫腹腔镜联合手术组（8例），比较两组的治疗效果。结果阴道三维超声引导阴式手术组的手术时间、住院天数短于宫腹腔镜联合手术组，术中出血量、治疗费用少于宫腹腔镜联合手术组，差异有统计学意义（P＜0.05）。两组的总有效率比较，差异无统计学意义（P＞0.05）。结论阴道三维超声引导下阴式手术和宫腹腔镜联合手术治疗剖宫产术后切口瘢痕憩室均能取得良好效果。对于临床医疗条件和技术力量相对薄弱、人民生活水平偏低的基层地区，阴道三维超声引导下阴式手术更值得推广。

Abstract：ObjectiveTo compare the clinical effect of transvaginal three-dimensional ultrasound-guided vaginal surgery and hysteroscopy combined with laparoscopy in the treatment of incisional scar diverticulum after cesarean sec tion,so as to provide reference for clinical treatment.MethodsAltogether 46 cases of patients who were treated with repair of incisional scar diverticulum after cesarean section from June 2012 to December 2016 in People′s Hospital of Baiyun District were selected as the object of this research and divided into transvaginal three-dimensional ultrasound-guided vaginal surgery group(38 cases)and hysteroscopy and laparoscopy combination group(8 cases)according to the different surgical methods.The treatment effect was compared between the two groups.ResultsThe operation time and length of hospital stay of transvaginal three-dimensional ultrasound-guided vaginal surgery group were shorter than those of hysteroscopy and laparoscopy combination group,intraoperative blood loss and treatment costs of transvaginal three-dimensional ultrasound-guided vaginal surgery group were less than those of hysteroscopy and laparoscopy combination group,with significant difference(P＜0.05).The total effective rate of the two groups was not statistically significant(P＞0.05).ConclusionCombination of transvaginal three-dimensional ultrasound-guided vaginal surgery and hysteroscopy plus laparoscopic surgery can achieve good results in treatment of incisional scar diverticulum after cesarean section,combination of transvaginal three-dimensional ultrasound-guided vaginal surgery is more worthy of promotion for the areas with poor clinical medical conditions,relatively weak technical strength and low living standards.
