卵巢成熟性囊性畸胎瘤合并黏液性交界性肿瘤的临床病理观察

摘要目的观察卵巢成熟性囊性畸胎瘤合并黏液性交界性肿瘤的临床病理特征。方法将2001年1月～2016年12月我院收治的20例卵巢成熟性囊性畸胎瘤合并黏液性交界性肿瘤患者纳入本研究，对其临床资料进行回顾性分析。分析总结患者的临床病理特征，探讨其转归预后情况。结果卵巢成熟性囊性畸胎瘤合并黏液性交界性肿瘤的发病以中年人群为主；实验室检查可见血清肿瘤标志物增高；盆腔CT检查可见卵巢存在囊实性包块，肿瘤界限清晰；巨检可见肿瘤局部存在破口，破口处有少许黏液样物质流出，肿瘤切面为多房、囊实性包块，囊性部分含有黏液样物质，实性部分呈灰白色；镜检可见囊腔内存在黏液性上皮细胞，卵巢间质浸润，部分区域存在无间质乳头样变，腺体共壁形成筛孔状，高倍镜下可见上皮细胞分化良好，呈高柱状，细胞质内存在黏液，细胞核增大且存在核分裂象；免疫组化检测可见畸胎瘤上皮CK7呈阴性，CK20呈局灶性阳性，黏液性交界性肿瘤上皮CK7呈弥漫阳性，CK20呈局灶性阳性。所有患者均接受肿瘤切除手术治疗，治疗后3个月未见复发。结论卵巢成熟性囊性畸胎瘤合并黏液性交界性肿瘤在临床上相对少见，可通过血清肿瘤标志物、CT以及宫腔镜检查对其进行鉴别诊断，并实施肿瘤切除手术治疗，预后相对良好。

Abstract：ObjectiveTo observed the ovarian mature cystic teratoma with mucinous borderline tumors of the clinical and pathological features.MethodsTwenty patients with mature cystic teratoma with mucinous borderline ovarian tumors from January 2001 to December 2016 in our hospital were included in this study,clinical data of patients were retrospectively analyzed,the clinical pathological features were analyzed and summarized,and the prognosis were also discussed.ResultsThe age of onset of ovarian mature cystic teratoma with mucinous borderline tumors in middle-aged people;laboratory examination showed that serum tumor markers increased;pelvic CT examination showed ovary was cystic mass,clear tumor boundaries;macroexamination visible tumor local break,break with a little mucus like substance out of the cross section of the tumor were multilocular cystic.Solid mass,cystic part containing mucus like material,the solid part was white;microscopic examination showed cystic cavity was mucinous epithelial cells,ovarian stromal invasion,there was no interstitial nipple like part of the region,the formation of common wall hole shaped sieve glands,epithelial cells could be seen at high magnification of good.A tall columnar cytoplasmic mucin and mitotic nuclei increased;immunohistochemical detection of visible teratoma epithelial showed CK7 negative,CK20 was positive in focal,mucinous borderline tumors of epithelial,CK7 showed diffuse positive,CK20 showed focal positive.All patients underwent tumor resection,and without recurrence 3 months after treatment.ConclusionOvarian mature cystic teratoma with mucinous borderline tumors is relatively rare in clinic,differential diagnosis can be carried through the detection of serum tumor markers,CT and hysteroscopy,and with relatively good prognosis after tumor resection.
