SuperPATH入路与后外侧入路行人工全髋关节置换术的对比观察

摘要目的探讨SuperPATH入路与后外侧入路行人工全髋关节置换术的对比效果。方法分析2014年12月～2017年12月我院收治的100例人工全髋关节置换术治疗患者的临床资料，依据入路方式将其分为后外侧入路组（n=50）和SuperPATH入路组（n=50）。比较观察两组患者的手术时间、术中出血量、切口长度、术后功能锻炼时间情况及术后不同时间点起立行走测试（TUG）情况。结果SuperPATH入路组患者的手术时间长于后外侧入路组，术中出血量少于后外侧入路组，切口长度、术后功能锻炼时间均短于后外侧入路组，差异有统计学意义（P<0.05）。两组患者术后5、14、30、60 d的 TUG均明显短于术前，差异有统计学意义（P<0.05）；SuperPATH入路组患者术后 5、14、30、60 d的TUG均短于后外侧入路组，差异有统计学意义（P<0.05）。结论 SuperPATH入路行人工全髋关节置换术，患者的创伤小，术后恢复快，值得临床推广应用。

Abstract：Objective To investigate the comparison effect of total hip arthroplasty by SuperPATH approach and posterolateral approach.Methods The clinical data of 100 patients undergoing total hip arthroplasty treated in our hospital from December 2014 to December 2017 were analyzed.They were divided into the posterolateral approach group (n=50)and the SuperPATH approach group(n=50)according to different approach methods.The operation time,intraoperative blood loss,length of incision,postoperative functional exercise time and timed up and go test(TUG)at different time points after operation were compared between two groups.ResultsThe operation time of the patients in the Super-PATH approach group was higher than that in the posterolateral approach group,the intraoperative blood loss was less than that in the posterolateral approach group,and the length of the incision and the postoperative functional exercise time were lower than those in the posterior lateral approach group,with statistically significant difference(P<0.05).The TUG of patients in two groups after operation at 5,14,30,60 d were significantly shorter than that before operation,and the differences were statistically significant(P<0.05).The TUG of patients in the SuperPATH approach group after operation at 5,14,30,60 d were significantly shorter than those in the posterolateral app roach group,and the differences were statistically significant(P<0.05).Conclusion Total hip arthroplasty by Super PATH approach has the advantages of minimal invasion and rapid recovery,which is worthy of clinical promotion and application.
