床旁血液滤过治疗急性心力衰竭并急性肾衰竭的临床效果

摘要目的探讨床旁血液滤过治疗急性心力衰竭并急性肾衰竭的临床效果。方法选取我院心内科2015年1月～2017年1月收治的68例急性心力衰竭并急性肾衰竭患者作为研究对象，按随机数字表法分为观察组和对照组，各34例。对照组给予常规治疗，观察组给予紧急床旁血液滤过治疗，比较两组治疗后心率（HR）、呼吸频率（RR）、平均动脉压（MAP）、动脉血氧分压（PaO2）、血肌酐（Scr）、尿素氮（BUN）、钾离子（K+）、碳酸氢根离子（HCO3-）浓度变化以及治疗期间并发症发生率、病死率及心功能改善情况。结果与对照组相比，观察组的MAP、HR、RR、PaO2均有所改善，差异有统计学意义（P＜0.05）；观察组的BUN、Scr、K+浓度较对照组明显下降，HCO3-浓度明显升高，差异有统计学意义（P＜0.05）；观察组的并发症发生率及病死率均明显低于对照组，差异有统计学意义（P＜0.05）。结论床旁血液滤过对急性心力衰竭并急性肾衰竭疗效显著，值得临床推广。

Abstract：ObjectiveTo investigate the clinical effect of bedside hemofiltration in the treatment of acute heart failure complicated with acute renal failure.Methods68 patients with acute heart failure complicated with acute renal failure who were admitted to the department of cardiology in our hospital from January 2015 to January 2017 were selected as the study subjects.According to the random number table method,the patients were divided into the observation group and the control group,with 34 patients in each group.The control group was given routine treatment,and the observation group was given emergency bedside hemofiltration therapy.Heart rate(HR),respiratory rate(RR),mean arterial pressure(MAP),arterial oxygen pressure(PaO2),serum creatinine(Scr),blood urea nitrogen(BUN),potassium ion(K+),bicarbonate ion (HCO3-)concentration changes after the treatment,as well as the incidence rate of complications during treatment,mortality rate and improvement of cardiac function were compared between the two groups.ResultsCompared with the control group,the level of MAP,HR,RR and PaO2in the observation group were all improved,the difference was statistically significant(P＜0.05).The concentration of BUN,Scr and K+in the observation group was significantly lower than that in the control group,and the concentration of HCO3-was increased,with significant difference (P＜0.05).The incidence rate of complications and the mortality rate were lower in the observation group than that in the control group,with significant difference (P＜0.05).ConclusionBedside hemofiltration is effective in acute heart failure complicated with acute renal failure,which is worthy of clinical popularization.
