未婚女性重复人工流产的影响因素

[摘要]目的 通过调查研究未婚女性重复人工流产现状及其影响因素,探讨降低重复人工流产率的策略。方法 选取2015年6月~2016年5月于广州医科大学附属第三医院及广州市妇女儿童医疗中心自愿要求行人工流产术的494例未婚女性进行描述性问卷调查并追踪。根据1年内是否再次发生人工流产分为重复人工流产组和非重复人工流产组,应用方差分析及多因素Logistic回归分析研究未婚女性发生重复人工流产的影响因素。结果 未婚女性发生重复人工流产者91例,比例为18.42%。与未发生重复流产者比较,重复人工流产的职业因素、教育程度及生殖健康知识了解程度差异有统计学意义(P<0.05)。多因素Logistic回归分析显示,教育程度及生殖健康知识了解程度是未婚女性发生重复人工流产的独立影响因素,差异均有统计学意义(P<0.05)。结论 未婚女性重复人工流产发生率较高,影响其重复人工流产的危险因素为教育程度低和生殖健康了解程度低,应加强对该群体生殖系统健康的宣传教育,尽量降低其重复人工流产率。

计划外妊娠是全球范围内主要的性生殖健康问题,由于计划外妊娠者50%都选择了人工流产以终止妊娠[1],导致人工流产手术量居高不下。据世界卫生组织初步估计,全球每年有(4~6)千万女性接受人工流产[2],其中不少是重复人工流产,北欧重复流产所占比例为 30%~38%[3],美国接近 50%[4],而在中国,这一比例超过50%[5]。由于人工流产术有增高再次妊娠时早期流产率[6]及早产率、降低新生儿体重的风险,且风险随流产次数增加而增大[7],因此,人工流产对育龄女性生殖健康危害很大,且重复人工流产危害更广,对未婚女性生理、心理、经济均构成巨大挑战。本研究旨在探讨未婚女性重复人工流产的高危因素,从而寻找突破口以降低未婚女性重复人工流产发生率,改善其生殖系统健康状况。
1 资料与方法
1.1 一般资料
本研究于2015年6月~2016年5月在广州医科大学附属第三医院与广州市人口和计划生育技术服务指导所进行,调查对象均为因计划外妊娠而自愿选择人工流产的健康早孕未婚青少年女性(孕6~10周),自愿接受问卷调查,并进行为期1年的随访,共征集508名。本研究经广州市人口和计划生育技术服务指导所伦理委员会批准,所有调查对象均签署知情同意书。
1.2 方法
对纳入研究的508例早孕患者信息进行统计整理,由专人负责将患者的姓名、年龄、籍贯、职业、有无保险、教育程度、流产经历、是否麻醉、有无规律体检、初次性生活年龄、首次性生活意愿、是否多性伴侣、同居时长、采用何种避孕方法、性生理健康知识掌握程度录入Excel表格,录入方式采用双人录入,保证信息的真实可靠。术后1年由专人电话随访研究对象是否再次发生人工流产。
1.3 统计学方法
将Excel表格数据导入SPSS 17.0统计学软件进行处理,计数资料以百分率表示,采用χ2检验,采用多因素Logistic进行回归分析,以P<0.05为差异有统计学意义。
2 结果
2.1 未婚女性重复流产的单因素方差分析
本研究共收集到有效信息494例,其中重复人工流产者91例,占18.42%;非重复人工流产者403例,占81.58%。两组的职业因素、教育程度及生殖健康知识了解程度比较,差异均有统计学意义(P<0.05)。两组的年龄、户籍、是否有生育保险、是否常规体检、有无流产经历、是否麻醉下施术、初次性生活年龄、首次性生活意愿、是否多性伴侣、同居时长以及采用何种避孕方法比较,差异无统计学意义(P>0.05)(表1)。

2.2 未婚女性发生重复人工流产的多因素Logistic回归分析
以患者有无发生重复人工流产为因变量,以职业因素、教育程度及生殖健康知识了解程度为自变量进行多因素Logistic回归分析,发现教育程度及生殖健康知识了解程度是未婚女性发生重复人工流产的独立影响因素,差异均有统计学意义(P<0.05)(表2)。

3 讨论
Lv等[8]的研究提示,重复人工流产会影响雌鼠生育能力,在随后的妊娠中易出现妊娠失败、自发性流产、新生鼠体型偏小及新生鼠体重受影响,甚至能导致中孕期胎盘功能的代谢相关基因表达改变,而在人类也已发现类似情况。人工流产与各种妊娠并发症相关,对再次妊娠的影响如前置胎盘、早期流产、早产、分娩延迟、大出血、胎儿畸形、新生儿低体重甚至围生儿死亡[9-10],且风险与流产次数成正比,远期并发症则如盆腔炎、不孕症、异位妊娠等[11]。这些都严重危害育龄女性生殖健康甚至造成未来不良妊娠结局。
本研究结果提示,参与调查的未婚女性重复流产率为18.42%,略低于既往报道[12-15],可能与两家医院常规宣教,导致了较低的重复人工流产率有关。众所周知,社会分配的不平等决定了群体健康的不平等,有研究提示,出生地[16]及教育程度[17-18]是群体健康不平等的影响因素,尤其是生殖系统健康[19]。Fisher等[20]的研究提示,移民妇女重复流产率比本地妇女高出83%。Zurriaga等[21]的研究提示,在西班牙也存在这种情况。Makenzius等[22]的研究提示,在瑞士低学历者重复流产率高出50%。然而,对本地妇女的人工流产率及重复流产率起到保护作用的教育对于移民妇女却失去了保护作用[23-24]。本研究未发现出生地对重复人工流产率的影响,分析认为出生地对重复人工流产率有影响的研究均出自移民妇女、文化差异巨大、安全感缺失严重,而本研究中外地人并无此情况。此外,本研究中教育程度与重复流产率相关,与相关研究一致。本研究结果还显示,生殖健康了解程度也与重复人工流产率关系密切,考虑到教育程度高者对生殖健康更重视,了解程度也更高,更明白避孕药具使用的重要性及正确的使用方法,即使之前有错误认知,经过宣教后的理解及接受程度也更高,从而表现出更低的重复人工流产率。
对于所有女性,尤其是未婚女性而言,安全有效的避孕技术早已有定论,然而在发展中国家,受主流社会准则所限,未婚青少年往往没有合理渠道去了解学习现代避孕技术,对避孕方法的使用容易出现错误,因而计划外妊娠高发,而有过一次意外妊娠经历的未婚女性很容易迅速重复妊娠,由于第二次怀孕仍然是计划外的,因此重复人工流产也高发。本研究并未发现避孕方法的使用与重复人工流产率相关,因能正确使用避孕药具者较少发生计划外妊娠,参与本研究的行人工流产患者绝大部分都没有足够的避孕常识,而本院宣教程度是相同的,以致参与研究者获取的避孕理论并无差异,故而导致本研究中避孕方法的使用与重复人工流产率无关。
有研究显示,女性性伴侣的数量也与重复流产率有关。拥有两个及以上性伴侣者重复流产率则比单一性伴侣者高出4倍[25]。本研究中,青少年重复流产率与初次性生活年龄以及性伴侣数量并未发现明显相关性,分析认为,本研究对年龄范围划分过广,可能影响结果,而基于中国传统道德观念,多性伴侣行为并非主流道德观,本研究共有12名参与者承认有多名性伴侣,不排除有故意否认者存在从而影响研究结果。
综上所述,教育程度低和生殖健康了解程度低是影响重复人工流产率的独立因素,而教育程度无法更改,应大力推进生殖系统健康教育,加强避孕知识的宣教;公众对避孕知识的启蒙可以从妇科和产前门诊开始,甚至可以扩展到其他诊所让男性也参与进来,使得避孕知识尽可能地普及,增加避孕药具的使用率;应对未婚者提供友好的、符合其心理特点的性教育及生殖避孕服务从整体上改善未婚女性生殖系统健康。

Influence factors of repeated induced abortion in unmarried women

[Abstract]Objective To study the current situation and influencing factors of repeated induced abortion in unmarried women by investigation,and to explore strategies to reduce repeat induced abortion rate.Methods A total of 494 cases of unmarried women undergone induced abortion were investigated and tracked from June 2015 to May 2016 in our two hospitals.According to whether there were recurrent induced abortion in 1 years,they were divided into the repeated induced abortion group and the non repeated induced abortion group.The influence factors of repeated induced abortion were analyzed by variance analysis and multiple factors Logistic regression analysis.Results A total of 91 cases of recurrent abortion were found in unmarried women,with a ratio of 18.42%.There were statistically significant differences in occupation factor,education degree and knowledge of reproductive health compared with the two groups(P<0.05).Multifactor Logistic regression analysis showed that the degree of education degree and reproductive health knowledge level were independent risk factors of repeated induced abortion,the difference was statistically significant(P<0.05).Conclusion The incidence of repeated induced abortion is higher in unmarried women.The risk factors of repeated induced abortion are low education and low understanding of reproductive health.It is necessary to strengthen the publicity and education of the reproductive system of the population and reduce the rate of repeated induced abortion as far as possible.
