急性心肌梗死患者应用主动脉内球囊反搏治疗后血小板减少的临床分析

Abstract Objective To investigate the causes of thrombocytopenia in patients with acute myocardial infarction using intra-aortic balloon pump (IABP).Methods Clinical data of 120 patients with acute myocardial infarction using IABP who were admitted to Shanxi Provincial Cardiovascular Disease Hospital from December 2017 to February 2020 were retrospectively analyzed.The time of application of IABP and the changes of platelets after application were recorded.The relationship between thrombocytopenia and age, sex and balloon size, and the influence of thrombocytopenia on the duration of IABP use and mortality in hospital were analyzed.Results The duration of IABP application ranged from 1 to 52 d, with an average of (10.04±8.04) d.After IABP treatment, 58 patients (48.3%) developed thrombocytopenia, and the mean time of minimum platelet count was (3.45±1.30) d.Among them, 51 cases were mild, 5 cases were moderate,and 2 cases were severe.The proportion of 40 CC balloon in patients with thrombocytopenia (87.9%) was higher than that in patients without thrombocytopenia (72.6%), and the difference was statistically significant (P＜0.05).There were no significant differences in age, gender and duration of IABP between patients with thrombocytopenia and patients without thrombocytopenia (P＞0.05).There was no significant difference in mortality between patients with thrombocytopenia (31.0%) and patients without thrombocytopenia (24.2%) (P＞0.05).Conclusion Thrombocytopenia in patients with acute myocardial infarction treated with IABP is mainly related to balloon size, and thrombocytopenia does not increase the mortality of patients.
