Analysis of risk factors of central venous catheter infection in maintenance hemodialysis |

Abstract Objective To explore the risk factors of central venous catheter infection in maintenance hemodialysis.Methods A total of 200 cases of maintenance hemodialysis patients with central venous catheter in the Fifth People′s Hospital of Shenyang from January 2018 to June 2020 were selected as the research objects, and the clinical data of patients were retrospectively analyzed.The incidence of central venous catheter infection and the distribution of infection strains were recorded.The risk factors of central venous catheter infection in maintenance hemodialysis were analyzed.Results Among 200 patients, the incidence of maintenance hemodialysis complicated with central venous catheter infection was 11.50%.A total of 19 strains were detected, among which gram-negative bacteria accounted for 57.89%, gram-positive bacteria accounted for 31.58%, and fungi accounted for 10.53%.The results of univariate analysis showed that there were no significant differences in gender and hypertension between the infected and uninfected groups (P>0.05).Comparison of age, catheterization site, puncture times, catheterization time, diabetes mellitus and anemia between the two groups, the differences were statistically significant (P<0.05).Multivariate analysis revealed>60 years old(β=6.229, OR=1.276, 95%CI=1.027-1.495), femoral vein catheterization (β=6.293, OR=1.498, 95%CI=1.142-1.832), number of punctures≥3 (β=5.852, OR=1.347, 95%CI=1.016-1.416), catheterization time ≥14 d (β=6.173, OR=1.132, 95%CI=1.045-1.521), with diabetes (β=5.982, OR=1.762, 95%CI=1.114-1.895), and with anemia (β=5.734, OR=1.358, 95%CI=1.126-1.573) were independent risk factors (P<0.05).Conclusion The risk factors of central venous catheter infection in maintenance hemodialysis include older age, femoral vein catheterization, more times of puncture, longer catheterization duration, and combined with diabetes mellitus, anemia and other basic diseases.Therefore, targeted nursing should be carried out for these risk factors in order to reduce the incidence of infection.
