[Abstract]Objective To systematically evaluate the effect and safety of Wenxin Granules combined with Metoprolol in the treatment of ischemic arrhythmia.Methods The literatures related to Wenxin Granules combined with Metoprolol in the treatment of ischemic arrhythmia published on China National Knowledge Infrastructure（CNKI），Wanfang Digital Periodical Full-text Database，Weipu Database，Chinese Biomedical Literature Service System，PubMed and Cochrane Library from the establishment of the database to June 2019 were searched.Cochrane evaluation method was used to evaluate literatures and Review Manager 5.3 was used for Meta-analysis.Results A total of 1752 patients with ischemic arrhythmia were included in 19 studies，including 877 cases in the treatment group and 875 cases in the control group.The total effective rate of the treatment group was higher than that of the control group，and the difference was statistically significant（RR=1.21，95%CI [1.17，1.26]，P＜0.000 01）.The incidence rate of adverse reactions in the treatment group was lower than that in the control group，and the difference was statistically significant（RR=0.41，95%CI [0.26，0.65]，P=0.0002）.Conclusion Wenxin Granules combined with Metoprolol has better effect in the treatment of ischemic arrhythmia，and it can reduce the incidence rate of adverse reactions and gain high safety.
