社区综合康复干预在慢性阻塞性肺疾病稳定期患者中的应用效果

Abstract Objective To explore the application effect of community comprehensive rehabilitation intervention on patients with chronic obstructive pulmonary diseases (COPD) at a stable stage.Methods A total of 84 patients with stable COPD admitted to our hospital from January 2017 to January 2018 were enrolled as subjects.They were divided into observation group and control group by random number table method, 42 cases in each group.In the control group, the patients received routine outpatient rehabilitation.In the observation group, the patients received face-to-face or telephone follow-up for community comprehensive rehabilitation, including health education, persuasion to quit smoking,oxygen therapy intervention, respiratory training, nutritional support, psychological intervention, symptom recognition and medicinal guidance.The hospitalization rate, lung function index and quality of life score were all compared between the two groups after 12 months.Results After the intervention, the rehospitalization rate of the observation group was lower than that of the control group with a statistical difference (P＜0.01).There were no significant differences in the terms of first-second forced expiratory volume (FEV1), percentage of FEV1 in forced vital capacity (FVC) (FEV1/FVC) and peak expiratory flow (PEF) between the two groups before intervention (P＞0.05).After intervention, the levels of the FVC, FEV1, PEF indicators in the observation group were higher than those of the control group, the differences were statistically significant (P＜0.01).There were no significant differences in the scores of life quality, such as ability of daily life, ability of social activities, psychological state of depression and psychological state of anxiety between the two groups before intervention (P＞0.05).The scores of life quality, such as ability of daily life, ability of social activities, psychological state of depression and psychological state of anxiety in the observation group were lower than those of the control group, the differences were statistically significant (P＜0.01).Conclusion Community comprehensive rehabilitation intervention can reduce the rehospitalization rate, and improve the lung function and the quality of life of patients, which is worth promoting.
