| Application of chronic disease self-management program in nursing care of patients with chronic kidney disease |

Abstract Objective To explore application effect of chronic disease self-management program(CDSMP)in patients with chronic kidney disease(CKD).Methods A total of 110 patients with CKD treated in our hospital from June 2016 to June 2017 were selected and according to the number of admission time,the patients were divided into control group and study group.The control group was given routine nursing,while the study group was given CDSMP on the basis of the control group.The differences in health behaviors,health status and self-management ability between the two groups before and after intervention were compared.Results The scores of exercise and communication with physicians of study group were significantly higher than pre-intervention and control groups,and the scores of emotional management,social function,energy and self-management ability were significantly lower than those in the pre-intervention and control groups,the differences were statistically significant(P<0.05）.Conclusion CDSMP is helpful to improve the ability of self-managemen,health behavior and health status of patients with CKD,which is worthy of clinical application.
